The bottom of the well in OOT. Nothing else in that game bothered me, I was probably 7 or 8 at the time... hell the shadow temple is my favorite dungeon from that game. But that well... shit I still get chills when I play that part. Its not scary anymore... its just the memories of being scared.

The Sewer level, oh god those monsters scared me. And then I had to fight one giant one while underwater.
Second that, If it's the same game i'm thinking of, the weird slimey aliens with one eye?

Final Fantasy. The sudden screen flash and intense noise of a random encounter would make a friend and I jump and scream, but then we'd laugh like idiots.

The Haunted House level in Super Mario 64 was probably the thing that came closest to scaring me, but I have never really been easily scared, even as a kid. In fact, I found scary movies, books, and games quite interesting and intriguing. I was weird like that.

One of the first games I played alone was Shadow of the Beast on the Amiga. Scared the crap out of me.

The giant eel in the Dire, Dire Docks level of Super Mario 64 used to scare the shit out of me.

Resident Evil 2 for the playstation. All those zombies, and I think there was a giant alligator, it scared the hell out of me. I used to watch my brothers play it and I'd sometimes try it out, only to pause and run away when there were zombies in the police station.
I'm pretty sure this video game as a kid is what makes me so afraid of zombie stuff.

Super Metroid. The music and sound effects really got to me. My dad rented it for me when I was 6 and had to return it in the same day for an exchange.

I'm with the Shadows of the Empire, Sewer level crowd.

Something about fighting underwater has never quite set right with me.

I was introduced to games when I was *really* little, so I got scared by *anything* with scary music.
The one that comes to mind the most is Sonic 2, I loved the first level, but the underwater part of Chemical Plant Act 2 scared the bejeezus out of me with it's "you're gonna die any second now" music coming out of no-where.

On a more modern note I still have trouble playing the marine campaign on AvP1.
AvP2, no problem, but the Aliens in AvP1 are freaking scary, damn I love that game.
