Mine is actually the Yu Yu Hakusho game for the Gameboy. It was one of my few games and I was able to memorize just about everything that would happen in the game, even to a point where i memorized where and how many enemies would spawn. I loved that game.
Halflife 2. I've ran through it about 5 times - considering I rarely even replay a game, that's a lot. However, I did used to be obsessive with lylat wars, and I replayed that enough times to get every medal on expert mode.
I've probably played and replayed MGS2, MGS3:Subsistence, Resident Evil 4, Kingdom Hearts 1 and 2, and Final Fantasy X at least five times each. I loves me PS2.
Everytime you think about it, someone re-installs it.
As for me, I'd have to say The Legend of Zelda: Ocarina of Time. Second being Oblivion and third probably being Resident Evil 5. Actually, switch the last two. Played through RE5 so many times I could recite the dialogue. Bad, I know.
