Star wars Republic Commando hands down a good 15 times though not sure because i keep returning to play.
If you count every difficulty level as a game on its own... Man, it would be probably close to a hundred times, or more.dsmops2003 said:
Finished Devil Survivor on the DS three times, playing fourth time right now. Mass Effect three times, fourth now on Insanity. Also Neverwinter Nights few times. Persona 4 twice, accumulated over 200 hours on my save, with only 91% of Personae. Oh, and Pokemon Yellow/Red/Blue over 7-8 times, Silver/Blue about 4 or 5 times. Guild Wars - finished each campaign several times, the least played one for me is Eye of the North and I finished it four times. Factions at least 6, Nightfall about 9 or 10, Prophecies somewhere in-between.

Love that game. so many character to build a team with and they're all varied and interesting. Add on that your own personal information can alter stat growth and that you have a choice of which character interact that can lead to different endings.
A close runner up would be Sim City 2000. Every now and again I fire it up and try building a new city from scratch. Then set it on fire.
My wife makes it a point to play through Eternal Darkness at least once every year.

I've replayed Super Mario 3 at least four times that I can remember. Baldur's Gate: Dark Alliance is one I've at least played twice.
GTA: San Andreas. About 8 times I think.
And of course Kotor I - 4 times
Kotor II - same (played Kotor II first...)
Oblivion - well, if it's the main quest, then about 3 times, but I spent a fuckload of time messing around
Fallout 3 - 4, too
Mass Effect - 4 (you really should've seen this coming)
Mass Effect 2 - 3 (ha! gotcha!)
GTA IV - 4 times (say whatever you will)
GTA IV TLAD - twice, just to see the ending again
Planescape: Torment - am playing it now for the first time, playing (or reading? hehe) something with this good writing just feels right, godawful graphic notwithstanding
Assassin's Creed - 4 times (hey, I liked the game)
And there were a lot of other games surely, but I won't bother listing them.
