I quit 2 months ago. Pure cold turkey.
This, given I was on 20 Marlies a day for 17 years shows it can be done, but it's not easy.
Going through it step by step.
Remove every single piece of smoking ecoutrements as possible. That includes Rizlas, matches, flecks of tobacco, dog-ends(stogies?) because you will try and use them.
Soak all of them in water so you can't re-use them. (Try to make sure it's impossible to dry them out)
Eliminate all the anti-smoking leaflets you can (They just want to make you smoke again) and spray all your smoking areas with strong deodorants.
Now wait: Make up your mind to fully give up, totally. This may take a few cigarettes to lock in, so after every one, clean up again.
Then stop.
Now, you have two days of feeling like you're going to smash something and two weeks of feeling like a total failure to go through, so utilize any trick listed above to get you through that. I personally use a pint of water everytime you want a cig.
If you do pick up a cig again, slap yourself. Hard.
If you light it, light the wrong end.
If you inhale, make yourself throw up.
Your lungs will hurt, your senses will overwhelm you, you will start shaking and having headaches. Anything your body can do to slap you about.
And then, after about 14 days...

I can now walk into a room with people smoking and chat to them without making snide remarks or desperately craving one.
However, 2 things:
1) Don't become a smoke nazi: It's all too easy to justify horrific prejudice against these people so remember when you were like that.
2) You have stopped smoking: That doesn't mean the occasional one, or that one "just to get you through". Stopped. Finito. Quit. If you can write down exactly what the first 2 days felt like, all the better as it will remind you.
But, even if you've been stopped 40-50 years, there still will be that time when the sweet intake of air tempts you. Just listen to the mucous-swathed cough of someone afterwards first.
