The Blood Rayne Games, God of War games, F.E.A.R 2 was also pretty gory. Fallout 3 is really gruesome too especially if you have the Bloody Mess perk.

Probably the best game that uses gore and has a good scare factor is the Suffering games.

In the end it seems that gore is becoming more of the norm in games. If an M rated game doesn't have a few over the top deaths or gruesome segments its not an M rated game.

-Fallout 3. Headshots with the VATs system. So much detail was put into that.

-The Punisher. The interrogations were, putting it bluntly, over the top in their brutality. I didn't expect anything else, though.

-Call of Duty: World at War surprised me with the level of gore. Limbs flying, decapitations...

-GTA: Vice City. That was the first game I played where cranial explosions were possible. It shocked me then...

sorry, I meant arm there, got my words mixed up. Yes, I love ripping off someone's ARM and decapitating them with it. Honest mistake.

The Fallout series, when played on the highest level of gore and the Bloody Mess trait/perk, mmm...

Standing at point blank and firing a burst of rounds with the combat shotgun in someone's face in Fallout 1 & 2 was quite a spectacle, and I actually lol'd the first time I saw a "gore bag" in Fallout 3; that's just overkill(pun intended)

now i can't remember if it was the first one or whatever but soldier of fortune for the PC let you set pretty much anything in the options, including the number of ennemies that spawned, so setting difficulty to easy, spawns to ''ridiculous'' and moving around full speed with a shotgun gave you an average of 5 kills per second...and i mean that quite literally.
limbs and heads also came off and such...
anyway it may not have had the detailed gore style of Gears of War but the sheer amount of violence allowed per second of playtime was hilariously obscure, so i'd say it ranks pretty high on the gory games list.

I'd say Gears of War 2. Multiplayer is laden with brutal executions; singleplayer has massive amounts of blood at times (the protagonists nearly drown in it at one point).

Fallout 3 gore is hilarious.

But Gears of War 2 has better gore, and massive amounts of blood. Good combination.

Aside from that, I'd say Fallout 3 and Gears of War both contain a sizeable amount of gore.

Although, the Gears of War blood does kinda look like toothpaste...

On topic, I live in Australia, so I suppose the goriest game I've played would be something like dead space, or maybe a modded version of oblivion.

Painkiller. Hands down. How can you say anything else? the shotguns blows them to pieces and limbs are everywhere, blood has splattered the floor and light showers of pieces and bloody bits and colons happen.

coming Second would probably be Conan...no man should have that much blood..and you can split them in half etc. but it disappears too quickly.

coming third would be F.E.A.R
