DMC 3 gets my vote. That game was so damn fun to me, that the difficulty never frustrated me.

Gradius III. No, not the toned down version for the SNES, but the straight-from-arcade port on PSP. Also, screw you, crystal level.

Megaman is definately fair. Brutal hard at times, but it is possible to defeat that game without being hit. I just got 10 recently. So hard, but so rewarding. (so close to cracking my Wiimote in half! damn... stupid.. grrr!)

Really interesting to see what games haunt some people, like Castlevania... Having one recently got my first NES this past Christmas, I only really played it at friend's houses when I was younger, and in emulation. I never found it that difficult. I have one friend who will reset the game if he dies, because to play to win without losing a life is the only way he felt held the challange for him.

Fair is all in the eyes of the beholder. I can beat Mega Man 2 no problem, but Mega Man 9 is just dumb in my opinion. Its a great nostalgia trip, but the overuse of one hit death spikes and stupid placement of enemies and more death spikes is just more frustrating than it is fun. I'd love to enjoy the game, but only beating 4 bosses before I just have to give up since the levels kick my ass and then the boss kills me in 3 shots isn't exactly what I'd call fair...or fun.

Rainbow Six Vegas(the first one) on the hardest difficulty was pretty hard.
It was actually hard on the normal difficulty.

And I've played harder games, I just can't remember their names.

The Stalker games on Veteran difficulty. You pretty much die from one bullet in the beginning armor.

Ico and Shadow of the Collosus are games that you truly have to be intelligent to win, and every time i failed in something i knew it was my fault.

Demon's Soul's difficulty is overrated, yeah it is hard but it's not impossible, just difficult.

Ninja Gaidan could get rough. Also Dragon Age on nightmare, unless you know the game well enough to plan your party right I think some fights would be impossible.

It's funny that the OP listed games that are nearly unwinnable by design.
I Want To Be The Guy is designed to be as unfair as possible.
NES games have always been extremely hard, usually by throwing 50 enemies at your instakilled ass. Hell, a list of somewhat "hard but fair" NES games would end after three games(that I know of) Mega Man, Final Fantasy and Zelda.

On-Topic: On Normal mode, nothing is ever difficult for me, even Hard is relatively easy. Only Very Hard or higher are challenging, and then it's usually because the games get unfair by reducing your health to about 1/4th of what it was while giving enemies higher accuracy and sometimes even increasing enemy spawns.

So.. MGS4 and Painkiller are the only games I can think of where the highest difficulty is "hard but fair".

MGS4 is fair because nobody starts on Very Hard, but the only real change I remember is that enemies see you a lot better, they're a bit more accurate and can sometimes counter your CQC. So I guess Extreme difficulty simply increases the amount of training the enemy soldiers received.

Painkiller is fair because enemies no longer drop souls. You lose the 1 health increase per soul and the ability to turn in to an invincible demon. The problem is that turning in to a demon and destroying everything you see is fucking awesome, so playing on the highest difficulty isn't fun.

Unlike the original Gears of War's final boss on the Insane difficulty. It was so stupidly difficult, I still have no idea how I did it.

Battletoads. Beat it as a kid with my friend Adrian. We are no longer friends and I can't beat it anymore.
I'm willing to bet you did not beat Battletoads on two player mode though, since the Clinger Winger level has a fatal flaw if you're doing two player. That being said, my thoughts concerning Battletoads, "FFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFFF....."

Gears of War 2 on Insane was brutal.. but I pulled it through. (No, I did not screw myself over on the final boss. Shut up!)

The hardest one to this day -Final Fantasy XIII, first time playing it. Learning was.. slow in parts, quick in others. But the brokenness of scaling for spawns is just annoying, and I really don't appreciate being forced to grind for 30+ hours on one map just to be able to progress the story, thanks.

It was a bit of a whore to complete but I still loved every second of it.
Megaman 10 has been scaled down majorly in difficulty.

There was a fun flash game called 'the hardest game in the world', has a sequel now too. and its pretty hard. Not sure if this counts but for me i think it was a small man (one) made game called Gladius, an old school rpg that frowned upon saving/re-loading often and was just one huge underground dungeon, no towns or gold, use only what you found. Some classes were a bit harder than others.
