Open the gates of Ahn Qiraj in World of Warcraft. See, that can only be done once per server (and I don't even think it can still be done in Cataclysm), and to do that you have to go through a ludicrous amount of grinds, kills, quests, raids, etc. Then you have to be the first one on the server to do it or else all your work is for naught.

It's madness.

Any multiplayer achievement but the multiplayer has a very small or no community.

Halo Wars has one for reaching the highest rank online, but its next to impossible now that the community is small. And if you were to find a match its usually experienced players who rush.

WoW's Insane in the Membrane achievement. Maybe not the hardest as far as skill in gameplay is concerned, but the most time-consuming and soul-leeching, sure.

theres a trophy in gran turismo 5 for doing all the endurance events, theres one that is literally a 24 hour race on le mans, i image thats gruelling

"Start of a New Era" from Soulcaliber IV. Oh my god is that achievement hard to get.

OT: Well perhaps the one achievement from [Prototype] where you only kill (or is it consume) 4 cilivians throughtout the entire campaign. In a game like [Prototype] that's reeeeeaaally hard.

Although I imagine that the one achievement that the OP is talking about is hard. Me and my friends had to do that last level so many times due to how hard it was. And we'd always just rely on the elf to make it, fuck the warrior and wizard.

Its not that hard, in Atacama Desert Conquest infiltrate the enemy base, avoid to kill anyone and wait at the TOW launcher near the helipad, remember to stay hidden, only go for the TOW launcher if someone enters the heli. Its a bit of a dick move but oh well.
I guess that could work. That is if the enemy doesn't go for the helicopter immediately and circlestrafe you and your team to hell.

"Nothing Special" on Left 4 Dead. I dunno if there is some trick to it or something, but a whole campaign with no survivors taking any damage from any special infected? Especially with the population of morons in the game...

That one actually gets a lot easier once you get about 1/3rd of the way into the game. I found it hard as hell at first (I'm not particularly good at any kind of FPS to be honest) but once you get your hands on the decent weapons and plasmids, stock up on armour piercing ammo and research them a few times, the Big Daddies just become a minor inconvenience. If you're still struggling with it, here's a few tips:

Preparing for the fight:

1) If you have any Proximity Mines, launch them all onto an explosive container, then pick the container up with Telekinesis. Initiate the fight by flinging it at the Big Daddy for huge damage.

2) If you encounter a Big Daddy without a Little Sister, Hypnotise it if you can. Then go hunt down a Big Daddy with a Little Sister.

The fight:

1) Use Insect Swarm as soon as you initiate the fight. The bugs will make the Daddy occasionally stop to flail around.

2) After you've used Insect Swarm, switch to Electro Bolt for the rest of the fight, re-applying Insect Swarm as necessary.

3) Electro Gel from the chemical thrower will effectively lock the Big Daddy in place as it damages them. A full tank on hard mode will sap away 1/4 of its health while it stands there shaking helplessly.

4) Electric Buck in the shotgun is the next thing you should switch to after the gel. It'll stop the Big Daddy in his tracks for a moment after each shot, and if you fire all four shots rapidly, you can knock off another large chunk of its health before the fight has even really started.

5) After this is done, pour all of your armour piercing ammo into it, or use the Launcher and grenades or rockets. If you don't have the Launcher upgrade that protects you from splash damage, avoid using the Launcher on Bouncers. If there's a decent amount of distance and cover between you and a Rosie, the crossbow can deal pretty exceptional damage too, but be wary of its long reload times.
