When I play LoL/DotA, choosing to save an ally but lose a kill, or take the kill but lose an ally, can be a pretty tough choice. And its not tied to a good/evil spectrum, and is just part of the games mechanics and strategy :O! Plus you don't have time to think about it, you just have a split second to make your choices.

I haven't gotten round to playing FO:NV yet but I have played FO3, and this came as quite a surprise for me :\

For me, probably the decision not to kill myself when trying to complete the quest Are we there Yeti? [http://old.wowhead.com/quest=3783] before the Cataclysm expansion in World of Warcraft.

Mass effect 2, who to kill Samara or Morinth. It doesn't matter that Morinth is a sociopathic serial killer, but I still feel sorry for her.

Letting Meryll die in MGS so I could get the stealth suit.

I know it wasn't canon to the storyline, but I still felt bad throwing her to the dogs.

A lot of the parts of New Vegas. Do I rule myself and hope Yes Man doesn't betray me? The NCR are powerful and will come back, is stability worth the sacrifice of security and liberty? The Legion may not be that bad, we only see their war effort, what if they're telling the truth? Should I follow House, with his superlative knowledge and capabilities?

A lot of the faction oriented decisions were difficult, in the end I find House the most agreeable line, he simply knows too much for me to rule in his stead. Besides, fuck the BoS, they're technophilic terrorists, this is West Coast, not Lyons' little men.

Oblivion. Going into the Dark Brotherhood on my new file on the PC due to the inevitable murder in the Arena. I had to pause the game and think about it when given the proposal.

Eventually, I made my decision, and Midas Death Ray'd Lucien in the face as a very clear "No thank you".

I didn't find that one hard at all but there is a small side quest where two missiles are being shot, one targeting the colony and one targeting the defence and main infrastructure and you can only disarm 1.
that I found hard. kill hundreds of thousands of people but the planet remains viable for recolonization or saving the people who will then need to be relocated and moved off the planet because it can no longer sustain colonization.
That one I found a lot harder and it still stumps me whenever I play the game.

I actaully found that decision quite easy. The overseer of the pit seemed like a nice guy and was going only working the slaves so they could have a better future (the raiders are a bit nice if a little rough around the edges ounce you get to know them). It`s easie to fight a bunch of slaves than fully-equiped raiders too. Also when i went to Wherners(i think that`s his name if i remember correctly) i gave a chance to get out alive, and he just abadoned the slaves without a thought. And on closer inspection, he was going to steal the baby and dissect it anyways judging from all the surgical tools i saw lying around the cradle.
