I am going to go with Devil May Cry 3 (the original English version). I never got passed normal. I can't imagine Dante Must Die mode.
Actually Dante Must Die mode was the same in all versions of DMC3... but still pretty damn soul crushingly difficult... my inner child cries as he feels controller vibrate a third time, telling him he just died.
I got stuck in DMD mode on the enhanced version 'EXTRA BOSS' on the clown. I could not scratch the guy and those orbs were a huge pain in the ass.

I've played quite a few games and after playing them on Normal the first time through, the hardest difficulty didn't seem that bad afterwards, if anything, there were only slight changes, yes I did notice the difficulty change, but I wasn't really that bothered by it.

But what games have you played where the hardest difficulty made you think, "**** me, that was HARD, not sure if I'll be able to complete this any time soon".

I'm talking about the type of hard that used to exist in some games in the old days, not the hard where there are few saves and obvious game flaws, but the type of hard that makes you grit your teeth and clench your fists.
If you beat Touhou 6 on Normal with no continues, you're a f**king ninja.
If you beat Touhou 6 on Hard or Lunatic with no continues, you're a demi-god.

That game drives me nuts, and I don't know when I'll ever beat it. Although I've gotten really close to beating it without continues but even then I'm too worn out to even try it again.

Maniac Mode on Fire Emblem: Radiant Dawn. Make it past chapter six. I dare you.
Oh lord, the Dawn Brigade are so fantastically crap at the beginning...

I'd say Mount and Blade set to all the hardest settings would be pretty damn hard, though I haven't yet had the courage (or skill, for that matter) to set it anything past the default easiest settings.

Also, hell or hell mode on Devil May Cry (only played it on 4, wouldn't expect anything less of 3 though). Dear god.

I once tried playing Quake on Nightmare and I breezed through the first level.
Then came giants and flying shit and other things that wanted my head on a platter...
fuck.

Any of the more recent 2d Castlevanias, like Eccliessia, which I'm running through right now. Not only does it cause the enemies to do more damage, or gives you more of them, and more powerful ones earlier in the game, or in more asshole-ish positions, but it gives you a level cap: 1. For the entire game, you are trapped at level 1, no way around it.

WipEout HD Phantom on Elite difficulty. Or F-Zero GX on the hardest setting. Those story missions are a nightmare.

Fans of a good difficulty setting should try Dragon Age: Origins with the nightmare plus mod installed.

It does not just ramp up enemy HP etc, it rebalances a lot of the game to really make it interesting; eg. heavy armour takes a lot more strength to wear, the heal spell is not so ridiculously overpowered, 2 handed weapons do a lot more damage, and health potions do not act like magical arm regenerators, instead increasing your health regen rate.

If you found nightmare too easy, this is a really good way to play the game!

Dishwasher dead samurai on samurai difficulty. Tough shit. Play it alone.
This. That game is abusive, especially when you come up against some of the cheap enemies.

Call of Duty games are NOT HARD. Even on Veteran, they're so scripted, everything happens the same each time. Ergo, it's just about perseverance.

Ninja Gaiden on the Xbox has to come close to the top in hardness. That game is pure cruelty.

CoD 4 Veteran is pretty hard, but even it takes second to Halo: Reach on SLASO.

For anyone who doesn't know, this is Solo Legendary All Skulls On.

These skulls include, but are not limited to, the following: Health doesn't recharge without a melee. No HUD. Dying makes you restart the level. Shit like that.
If we're on the topic of SLASO, sure, Reach is tough, but nothing in the series compares to Halo 2 SLASO. It's extremely difficult but seems like it's probably theoretically doable; some levels haven't been done before, and some have only bee done segmented, like Gravemind [http://www.youtube.com/watch?v=tCHqiiRYj0M].

Its skulls can be more problematic. Sure, you get to have active camo instead of a flashlight even as the Chief thanks to the Envy skull. But the Covenant are ridiculously strong. Basically, for the particularly crazy stuff:
-Assassins ensures that most enemies are perma-cammo'd.
-The Armory skull makes it nigh-impossible to avoid detection by the enemy, and it makes the Covenant never sleep.
-Thunderstorm's rank-up causes all flood combat forms to have shields in addition to all the regular rank-ups.
-Angry causes the Covenant to shoot even more aggressively than normal.
-Ghost makes it impossible to cause enemies to flinch.
And that's on top of Halo 2's already ridiculous legendary.

SLASLO is horrific. Glad someone mentioned it. However, SLASO on Halo 3's Cortana level is painful. Those shooty Flood forms are fucking hateful.

I know right I cant belive it but I actually beat half them and did the first set of courses on the hardest difficulty. The rest I couldnt win first I think I got third on the second set but I couldnt even finish the rest the cars would make me explode before the end of the first lap.

Realistic mode in the orginial FarCry

Oh and Hardcore mode for Dead Space 2.
The human being that can complete a game like that at such a difficulty with only 3 saves (which, when faced with death, you return to instead of a checkpoint) must have a disturbingly impressive amount of time and dexterity to complete. I'm not sure If they are to be admired or exiled from humanity.

I dont know anybody whos managed to get over 9 kills and they were using items and managed to get a smash ball. Basically sonic is so fast plus there being 3 of you the oppoent just doesnt stand a chance there is always sombody attacking him or her.

That's Heaven and Hell mode.

Hell and Hell is you die in one hit but everything else is at DMD level IIRC.
