Any achievement that requires you to be online is going to be a ***** to get. Being at the top of the leader boards, winning a certain number of online matches etc. On their own they can be annoying, but when you add a story or one player mode they just because down right cruel.

Hands down the "There Can Be Only One..." Achievement for Total War: Shogun 2. It requires you to be number one on the multiplayer leaderboard, and only one person has ever got it so far.
This.

Seriously, how many people did they expect to get this achievement?

Oh, and I can't forget the achievement for getting an A ranking on the last level of Ikaruga on hard:

Easily "You can be my wingman anytime" for BF3. It requires you to finish Going Hunting with a perfect run. I take no damage and complete all the tasks but I still don't get it.
You have to also NEVER miss with any of your missiles launched at the enemy fighters. After you do that, the trophy/achievement will pop when you hear the BF3 theme again.

Can't believe no one has mentioned this one. How about the "Mile High Club" achievement from good ol' CoD4: Modern Warfare. Completing the bonus airplane mission on Veteran. To this day that's the only achievement I consider worthy of bragging about...even though 90% of getting it will boil down to pure luck.

For me, it was the trophy for getting to round 5 of Five Finger Fillet in Rage. I spent so much time on that one frustrating challenge...
:p I was thinking about mentioning this one, but really it was easy after I decided to cheat. Get through the first 4 rounds easily (since they always go in the same pattern), however for the 5th round which is completely random, I hit pause after each stab, allowing me to see where the next stab would be. :3

Any of the achievements in Dead or Alive Volleyball 2. Contrary to popular belief, it's not a terrible game at all provided you go int with the right expectations.. but the Achievements are just pure insanity. All of them. It's a game I've played on and off for years now and I have yet to get a single achievement in it.

Also, achievements linked to multiplayer play are just pretty terrible/annoying in general. Many aren't so bad at a games initial release but even a couple months down the line most in most games end up pretty much impossible. It would be okay if it was a skill based issue or something like that, but they aren't.. they end up impossible simply because there's not enough people still playing the game (assuming there ever was at all) to complete them.

This is probably a piece of cake to do nowadays with all the welfare epics available since (I haven't played the game in years). On release of WoW Cataclysm, the one heroic challenge holding me back was the last fight in the desert city place. I can't remember exactly, but it was something like you had to get so many stacks of this debuff applied to you before killing the boss, and each time you'd be taking more damage than before. So you've got all these adds that you can't kill, because they're supposed to give you the debuff, and then there's knockbacks and shit and lightning flying around everywhere. And I was frickin healing.

I don't know if it counts, but the one you get for beating The Impossible Game, the game on XBOX Live indie games that has an awesome soundtrack

Fun little game, but gods it is hard!

Boyfriend has this, says don't use missiles, guns don't count if you miss but if you miss with a missile you fail it. He says it takes a while but it's pretty much guaranteed.
I don't play BF3 but I saw him do this the other day.

OT- Boyfriend and I are doing Co-Op legendary on Halo: Anniversary and we failed hard at Speed Reader. We'll probably give it another go soon but we got... flooded with flood. That level is fine if you take your time but seriously.

Oh I know what you mean.
I got that one by accident on my healer and everyone was gratzing me and telling me how hard it was. I like getting achievements by accident, there's a nice moment of `What did I get that for?`

The platinum trophy for Warhammer 40,000: Space Marine has some rather annoying trophies that you need to get first before you can get this one, here's a selection of previous requirements:
True Son of the Emperor: kill 40,000 enemies (all modes) - so grindy >.<
The Emperor Protects: beat the entire first part (~5 levels) of the game on hard without dying or restarting, in a single session.
The Keeper of the Armoury: Complete all the MP weapon challenges - this includes getting several dozen multikills with the worst weapon in the game (the vengeance (aka grenade) launcher). Try doing that while being spammed by Plasmacannons and Thunderhammers. Not fun.

Oh, there have been a LOT of trophies/achievements that I've looked at and gone, "Ha ha, not happening!" and thus haven't even bothered with them. So, hardest one I have gotten? Probably that "Time Travel Will Tell" from Black Ops Shangri-La, followed closely by "Cryogenic Slumber Party" from Moon. The reason "Time Travel" was harder was because it took four players working together, so my two friends and I had to drag a willing fourth player into the mix. And since he didn't care as much about the trophy as we did, it was hard to keep him from running off and ruining everything because he was getting bored.

Most annoying:

Hands down it was "Acquire Waffle Weapons" on Der Reise in World at War. I HATED this trophy. I became an expert on this map because I spent HOURS by myself constantly hitting the box, hoping it would give me the weapons. There is no skill involved in this trophy. There is no gameplay involved in this trophy. It is pure luck. You stand there and WAIT for the game to give it to you. And if the game doesn't want to give you the three weapons, tough luck. I was so relieved when I finally got all three. Any trophy that is based solely on the game and your luck, not skill, is a stupid trophy.

Grinding achivements (fuck you Gears of War, I'm barely 15% done for Serious 2.0)

Any achievements for completing (usually) the campaign on the hardest difficulty which is really "Enemies have 100% accuracy, the reaction-speed of the Flash, deal double damage, have double health, etc." mode.

World at War was just terrible with this, especially on the Heart of the Reich level. Infinitely respawning enemies is now a game devolping sin in my book.

I just got that achievement this summer. I celebrated by turning on cheat codes, setting the difficulty to recruit and just laughed as I mowed everything down with a silenced MP5.

Holy shit! That's just not fair.

OT: For me, it would have to be the trophy in Infamous where you have to complete all the stunts. I just don't have the skill to pull of sticking an enemy with a grenade while said enemy is in mid-air!

There's one for Star Ocean the Last Story where you're supposed to get all 900 in-game battle trophies. These combine the insanely hard (kill the final boss on hardest difficulty in one minute), the fantastic random number grind (hit for 7777 damage or leave an enemy with 6 health left), the absurdly tedious (kill thousands of enemies), and everything in between. Each of these terrible things appears several times over the 900 trophies you will be getting. Someone calculated that you should expect to spend at least 1000 hours doing this, despite the fact that it would only take a tenth of that to beat the game and its side quests

Man these are some intense games. My example isn't nearly as impressive. Personally, beating COD:WaW on veteran. Those who know of the misery only shudder...

Can't believe this hasn't been brought up yet (or I haven't seen it):

Little Rocket Man from Half-Life 2: Episode 2 requires you to take a garden gnome from the beginning of the game all the way to the end. For those who haven't played, you can't place this garden gnome in your inventory or whatever and forget about it for most of the game. You have to manually pick it up and carry it from start to finish, braving moments where it could get broken, lost, or falling out of a rickety car. Once you place it in the rocket, you better hope to God that the achievement pops up.

Also, another one is from Half Life 2: Episode 1, called The One Free Bullet, where you have to complete the game without using a single gun at all, save for a moment where you are required to shoot a lock off a door.

No, the worst was the fact that when you knocked a Nazi into last stand, he would become a grenade-lobbing turret until you finish him off, just chuckin' grenades at you every 5 seconds.

I tried The Emperor Protects. Died on the first level. Gave up. Seriously the achievements in that game can be brutal.

Neither of these were too challenging for me. The gnome is a pain in the ass but doable with some patience. As for the one bullet, I just turned the game down to easy and it was trivial.

Seriously...

10,000 kills on Gears of War. A lot considering it is elimination type gameplay (annex kills don't count). Also there was a factor that it wouldn't register right at 10,000 on the leaderboards as it seems to have a different tally. Might have something to do with disconnect kills not counting or something. Months of playing the game religiously finally registered at like 12,000 something kills.

Not hard per say since anyone who plays it could eventually get that many kills...but incredibly time consuming.

When it finally popped up I was yelling in joy at the top of my lungs at 3 am despite the other residents of the house being asleep. I was so happy I just didn't care.
