Well, in Zelda- twilight princess, just as you are reaching out to get the master sword..
MINIGAME TIME! Lets play some pointless, stupid, out-of-nowhere puzzle that take ages to finish properly. Damn you Nintendo!

Or game that use arbitrary barriers to slow us down, such as needing a key for a rotten wooden door when you carry C4.
NPCs that only can say 2-3 lines before looping and repeating the same stuff.
Ennemies (oblivion reference) that can eat 1000000 arrows in the face.. and not die.

When every damned event only happen because/for/are about YOU, and only YOU. Make some world where significant stuff happen and lesser problems can solve themselves PLEASE?
Dumb allied/ennemy AI in general.
NPCs that scream : QUEST/PLOT/WHATEVER please will someone (you) will help us?

I want to be able to say -no- to a quest, and soon after someone (npc) else takes the job and either succeed or fails it.

Having a rope switch bounce around a cave in Oblivion.
I'm not sure what happened but i couldn't get it to stop.

Also, the guards in Oblivion
You can murder someone to the deepest, darkest cave miles away from civilization and the guards will still know that it was you who killed them.

people talking to me and interrupting me obviously.
One example was when i was playing L4D2 with a mate in Hard Rain, there was a heavy storm in the game and mum started talking to me, i kept wondering how the hell she could hear me while a storm was going on, then i realised that it was a normal summer day in the real world.

And to be even more fair, the Assassin's Creed games aren't the only ones with the problem.
The most amusing of these I had on AC2 was when climbing a ladder to get to the rooftops. There was a group of thieves at the top and upon reaching the rooftops after my rather easy ascent, I was greeted with 'Wow! How did you do that? You make it look so easy!'

I was just playing Silent Hill 2 on the 360, and two major backwards compatibility glitches occured. On the CG cutscenes the sound and video was not in sync, and worse was that once I reached the labyrinth area the sound cut off for everything except the BGM and radio static for enemies; the gunshots, footsteps, and enemy noises just cut off which really diminished the experience, especially since this is my first time playing through the whole game in longer than a year.

When my mother interupts me every five minutes to do chores! Honestly, how am I supposed to get anything done!?! Oh, you meant in game...ummm, no not really.

Most games have really poor immersion to start with, but I find things like the following to really break up the immsersion, I'm listing the things because it happens too often:

* "Press X to to something" right in the middle of the screen almost covering up your crosshair, particularly annoying.
* Switching from 3rd person to first when "using" cover, eg: Vegas2
* Lag
* Frame skipping
* "Gamey" sounds, like those ones that come up when you get a challenge completed in Call of Duty
* Cluttered HUDs, see Call of Duty
* Timers
* Text popping up telling me useless information like score, time remaining, winning/losing etc.
* Boring games

I find most games have really poor immersion. ArmA2 for me was the least "immersion breaking", but even it had the odd annoying thing ruining the atmosphere.

Load screens (Half Life series in particular) or when my friends sign on to LIVE.

Achievements do get annoying when you get them every ten minutes for "first reload" or "five head shots". It eventually wears off thankfully after you start to gain more of them.

For everyone complaining about achievement notifications, I'm pretty sure there is an option to disable those. Though I don't find them to be annoying.

Sure they would, the next game starts with 'Oh shit, someone's let the big bad out again.' There's no reason it had to be you, in the same game you just went through hell and high water to seal him away in.

You could easily create another sequel off a game ending that way.
In the next game, the Prince finds that there is another big bad evil thing that is wrecking havoc and the only way to take him down after finding a bunch of scrolls and riddles and such... is to release the fcuker that you enprisoned and steal his spirit to empower your weapons to take on the new big bad dude.

But now there is an army guarding the place because they were mislead by the new big bad that the old fcuker's minions are trying to release him. Even when you release him and steal his soul, awakening the girl... the army sends their best to capture you and the girl and seal you into the prison because they think your possessed by the evil.

So now after you woke her up and stole the evil dude's soul, you not only have to take on a massive platforming but you got to do it under a time limit. If you take too much time trying to get past one section of platforming puzzle, guards will show up to attack you. Depending on how much time you waste depends on what guards show up. I.E. at first dudes with swords, then dudes with arrows or slings, then magi... etc.

Then of course you'll have the girl wondering whether or not you are possessed. if the new big bad dude is really real ... and to really piss you off, the harder modes will place a time limit during the boss battles as well.

send that to ubisoft XD
