You're not doing it right. Doesn't hurt that much and you have a good chance of incapacitating someone with a headbutt.

I did something like that, but instead of headbutting my knee I kneed my nose.

I headbutted somebody's fist once (I didn't have time to dodge, but had time to make sure the hardest part of my head was what was hit) and broke a few of their fingers. Good times.

Headbutted a twat from school once... Got in my face and said the infamous "Come at me bro"... So I headbutted him. I know I did it right because the pain was minimal and he was howling in pain (howling I guess? Not sure what the noise is called when their nose is injured)... The years of martial arts practice probably helped.
