My ideas:
-Tali is genetically male with gender dysphoria and is living a lie with the female suit and fake female voice. This is revealed and he becomes Shepard's same sex romance option.
-The Reapers true motivations are benevolent and they want to transport humans to a land of eternal pleasure or something like that.
I'd go on a tirade about how Bioware as a whole makes roughly two-thirds of your proposed lottery winning every year and that taxes would take roughly 20%-33% of that 40 million and that the head writer/developer of the team responsible for the game would probably be a well-respected/paid person, but that's not the point of this thread, I suppose.
So On-Topic:
As the writer:
Remove the "neutral" option from Shepard's dialogue options completely Remove all dialogue options completely, make every returning squad member the polar opposites of their established personalities, kill off Tali before ever revealing what Quarians look like, and have Shepard's only romance option be the absolute most annoying character on the entire team.
As a developer: Make the planet scanning option from ME2 an even larger part of gameplay this time around, and have combat go into a Final Fantasy XIII-esque "pocket dimension" where everything is done through menus and you have no actual input on what happens, remove all side-quests and make the core story half as long as ME2's was (which would be half-and-half writer/developer, actually).
I could probably think of a few more things, but that's it from right off the top of my head.

If I wanted to ruin ME3, I would make it so that near the end of the game you have to drive the Mako through space, ramping from asteroid to asteroid to get to the Reaper mothership so you can blow it up and save the universe.
Other then the Mako being incredibly hard to control and the jumps needing great skill to make, if you happen to miss and die during the mission, you get the bad ending where everyone in the universe but Joker dies and then your save files from Mass Effect 1, 2 and 3 are all erased.

Actually, after seeing this I'll add another thing to my "As a developer" part:
In addition to the planet scanning being a huge part of the galaxy exploration, let's say something around 45% of the "gameplay", another 45% will be driving the Mako around with even worse controls than from Mass Effect 1, and then the last
5% EDIT: hurk, 10% will be the aforementioned "pocket dimension" combat or just moving down linear corridors to the next plot point.
