Are they smaller blankets, though? Or do you go through the rigmarole of spreading standard-sized [that sounds offensive, but it wasn't intended to be] ones on the floor and working that angle?

Exactly this. I'm 6" exactly, and I have a king sized bed. Doing the doona (or duvet, as others call it), I still have to use weights to pin down corners as I crawl around on the inside. To put Duvet covers on more easily, place the duvet cover inside out over your arms, so that you are holding the outside corners with your hands. If I have to use ingenuity to do it, when I'm taller than average everywhere in the world (with the exception of the Netherlands, the freakish giants), then I don't expect it's much different for people considerably shorter.
