Three different occurrences come to mind for losing save files for me.
First was way back in the day playing through Super Mario 64, managed to get up to around 80ish stars, got bored/stuck so started a new file on one of the blank slots. Came back a few hours later after my brother had been playing to find all the files had been copied over with the new file with only 8 stars. As can be expected, me being a kid at the time there was rage involved.
Second is the countless times I've accidentally overwritten a FFIX save file with a new run through since when you choose a different load the save remains as default over the previous save file. So the need to save asap for whatever reason was followed several times by "bugger, I did it again didn't I?" :/
Finally is ylod on the ps3 which obviously lost all of the save data which included Fallout 3, Oblivion and Disgaea 3 for those with the most time taken to complete. Still technically have a Fallout 3 file which I saved a back-up of but haven't seen what happens when I load it as I saved directly infront of the point lookout dlc and don't have the means to re-download it...
Wasn't too fussed about losing the data as I'd manage to get to the end of the story for most of the games involved and Disgaea's post-game will just be left unexplored unless I get the urge to go through it all again.
