If what the OP described is accurate then it absolutely crosses the line, and I have little doubt that someone who would do that once without batting an eye has probably done it plenty before, and likely will again. So assuming again that his description of events is an accurate account, I'd report her. Doesn't matter if she's my sister, the kid's safety comes first. OP. Unless you think Apples life is at risk, I'd try to stay out of it if I was you.
But that's assuming he's accurately recounting it. But even if his description isn't what actually happened, but is skewed by his interpretation of events he didn't have a close up view of (and given eye witness testimony is notoriously unreliable), he should still report her. Let the social workers investigate and see if they find any reason to intervene further.
