If it was made, what old or new things should be put into it story wise or game wise.

Haven't you learned from the previous Episodes? Valve won't put in anything new gameplay-wise. It's because it's still Half Life 2.

It already is being made (though Valve have been notoriously silent on it).

To answer your question:

- Implementation of Portal elements (which was implied by Episode 2)
- How Alyx handles what happened at the end of Episode 2 (I expect at least a slight personality change Valve!)
- Continuation of the excellent level design
- More battles like the end of Episode 2
- Kill an Advisor with a crowbar
- Updated visuals
- Who is the G-Man?

I can think of more, but that's a good start.

-Cover based combat with ess emm gees.
-Freeman will be replaced with a large marine with biceps the size of his head.
-Alyx will find a way to always be wearing a bikini and her dialogue will be entirely double entendre.
-The G-Man will be a CIA sleeper agent and will join the new Freeman as they battle the advisor in a climactic boss fight.
-The Final boss will only be able to be killed by the American Army who fly in at the last minute.
-Graphics will become photo-realistic and the Source engine will be replaced with the Unreal engine.
-Level design will be handled by Activision.
-Barney will die
-Headcrabs will be replaced with attack dogs for better realism.
-The crowbar will be replaced with a knife, as it suits the character better.
-Eli will be found partially alive at the end fight, pumped up with steroid enhanced biceps and will blow himself up to save Freeman at some point during the fight.
-The HEV suit will be replaced with a bullet proof vest.
-Med kits will be replaced with regenerating health.

On topic, I'm not sure what they should add, maybe add more references to portal.

And ending that clearly states HL2 EP4... but it should be released sometime in this millenia to be something to be excited about.
