People would definitely be more careful. There would be no more MW2-style FPS games that had this feature; they'd all be slow-paced, tactical shooters. Realistically though, I probably wouldn't use it, however it was implemented. They'd have to scale down the pain level dramatically. And really, the only way a system like this would work is once we reach the stage where games plug right into our brain, over-riding our nerve signals so that we feel the dust kicking up in our face; our brain tells our body to move, but the signal is sent to the game instead.
I could go on about how secure servers would need to be, because if someone hacked it they could potentially send the wrong signals to the brain and kill players, and about all the ethical issues involved with the whole idea of it, but that's a discussion for another time.
