I'd get the Shinigami eyes, first of all. I really don't need half of my life, anyway, and it'd be really cool to see how long it'll be before someone dies. The name thing would be fun to use to screw with people.
After the eyes, I'd totally use it. I wouldn't go to either heaven or hell, but a lot of people here seem to be missing the fact that, in the world with the Death Note, there is no heaven or hell, so it wouldn't make a difference. I'd use it for personal gain, to get whatever I want. I'd make most of my victims die painlessly in their sleep. The conditions of death wouldn't be cruel; for example, Stephanie Meyer would make a public apology for starting this whole Twilight craze after realizing that her books were terrible. Then, she'd donate all of the profits to charity organizations. Satisfied and happy with her deeds, she'd die painlessly in her sleep a few days later.
Of course, this comes with the same moral dilemma as with the Geth in Mass Effect 2: is it morally sound to control someone's thoughts in order to MAKE them happy? Well, in this context, I think it wouldn't matter. Meyer would die anyway; I might as well make her feel good about herself. Even if this is something that she wouldn't do normally, she wouldn't realize. She wouldn't know. To her, these would be actions that she did of her own accord, and she'd feel truly happy about it.
Am I a monster? Probably. All I know it that I would enjoy having a Death Note quite a bit. Hello, population control!
