The swing glitch in GTA 4 is always a great way to die.. End up with your back broken along a skyscraper.
I agree, it's even better on TBOGT. If you wear a parachute and jump out just when you take off, you fly unbelievably fast like Superman for a short while before faceplanting whatever is in your path, I did it once and went straight into loads of boxes and trash cans, it was fucking hilarious.

I really like the mad deaths in Garry's Mod too, like being killed by a bathtub to the face from half way across the map. Or on MMOs where it's one versus 10, so you put like 20 healing spells on yourself, use every cooldown as well as every item in your inventory and just run the fuck in spamming AoE.

Planescape Torment

Using the Blade of the Immortal in the Negative Material Plane.

I almost expected one of those snide Half Life 2 death screens to come up, the ones which refer to the player's "astounding lack of judgement".

Recently I died in Dead Space 2 via getting stuck in the air-lock door and being torn in half, complete with intestines on the floor. I actually yawned because it takes like half a minute for him to actually end the animation and bloody die!

I think one of my favorite deaths was in L4D at Mercy Hospital. The Tank came and knocked me off...but then proceeded to follow with an elbow drop. XD

Yeh, I wasn't expecting people to die in games on purpose, (unless you enjoy to) that would be mean of me. But we will die in a game and I was wondering if any actually ammused you when you did =)

Anyway, I forgot my favourites.
Shooting the heads of the baddies from Jet Force Gemini (N64) and then collecting them for further insult.

The Cerebral Bore, Turok Rage Wars. Watching an enemies brains spill over the floor as you hear the blade saw through the skull. Erk. The fact it was the friendly N64 graphics at the time makes it not so horrifying. =)

Dead Space 1 and 2, and Resident Evil 4 and 5 got quite the lot of different cinematic deaths, even if they're not in cutscenes. Pretty nifty.

Also, even though I didn't die from it, I enjoyed my encounter with a German Pak 38 in Brothers in Arms: Earned in Blood. It's a mission near Baupte where Joe 'Red' Hartsock get's his ring-finger shot off (irony, seeing how he is the only married guy in the squad), and later on you meet a Pak 38 stationed near a church, and it has a long clear line of fire on you if you decide to charge it instead of flanking it. I left my squad behind, charged and the Pak 38 fired a shot that barely missed me. It took off maybe 80-90% of my health and sent me flying a meter or two. While I'm trying to get up while still shellshocked the Pak 38 fires another shot which goes over my shoulder and flies off in the distance. I continue, take out the crew with a grenade and finish the mission a few minutes later.

The really fun part? If you suffer damage in BiA: EiB, you do not regenerate, and there is no "medkits", and you see blod on your characters face and uniform. Also, after this mission there's a cutscene where one of 'Red's friends say "Jesus, Red! What the hell happened to you?!" (because of the finger he lost, not the blood on the face and all). I laughed :p

This is what happens when you take the Health Packs all the time.

My personal favorite fail-death in L4D was getting snatched by a Smoker, saved but then my friend gets Boomered and a Hunter pounces me instantly. Those AI zombies know the meaning of teamwork!

Also:

[sub]Forgot where you planted the little bastard, huh? Yeah, that'll never come back to haunt you...[/sub]

Fallout: New Vegas!

This was one of my funniest deaths (I assure you it is a good read):

Fallout: New Vegas!

Once I was walking through the mountains, when I came across a path through them. Wandered through with my companions when on the other side, was a Blind Deathclaw! Needless to say I died the first time...

Well the second time I was prepared, I planted 3 C4 in the pathway, then drew the deathclaws attention to me before running back to safety. When I blew up the C4, with the deathclaw on top of it, he went flying, like a ragdoll effect miles away, I thought for sure he would have died from that.

I continued walking, until I came across a red tick on my compass, I sprung round the corner to find the same Blind Deathclaw waiting for me, with 2 bars of health left! I then proceeded to die again :(

The Silent Hill 5 over-the-top death animations were awesome because I hated the MC XD
Same deal in Dead Space 1~

I also personally laughed my ass off when James Sunderland drove his lard ass into a lake during the In Water ending of Silent Hill 2

There was one time in Lord of the Rings Conquest, Evil Campaign final mission in the Shire, I was playing as Sauron, my friend was playing the Balrog, I was walking across the bridge when I noticed a massive eagle flying towards me, but I thought "Oh, it can't swoop me away. I'M SAURON!!" Two seconds later, "Let's do this-- AAAAAAAAAAAAAAAAAAAAAAAAaaaaaaaaaaaaa..."

My friend needed a minute to catch his breath from laughing so hard.

Well not putting on the space suit in Fallout 3's Mothership Zeta and then going out the airlock was pretty funny.

And while it wasn't me who died, launching my friend out of the level with Sledge's shotgun was immensly fun. Seriously, that gun can send a target flying. We began to get around using it.
