I have two good ones, the first is:

I was playing fallout new vegas for the first time and was fighting joe cobb and his powder gangers, I decided to throw a stick of dynamite using VATS when all of a sudden the game glitched and my character didn't throw the stick.......I sat there and watched the fuse ignite and my character lost his arms and head.

also:

In dead space two i was being chased by some necromorphs and thought it would be funny to shoot a window and kill them all.
I saw one of them being sucked into space and so I shot the lock to save myself.......two seconds later my arms head and legs were dismembered by the other necro that stayed inside because of a terrain feature.

One crashed computer.

I always liked dying in a big fiery explosion...

My personal fave was to pull frag grenades and charge people Kamikaze style on Modern Warfare... those 9 year old bastards never saw it coming...

There was this waterfall that I was going across when I was getting shot at, so I took out a grenade and as I was about to throw it I got shot. The grenade drops where I'm standing, blows up, throws me off the ledge and I fall to my death.

I think I'll describe my best game-death experience.
GTA3, Vice City. I was going on a city wide rampage to see how long I can keep this massacre going. For some reason, it never escalated to tanks/army so that was pretty awesome to keep the 'easy-to-kill' guys.
I started off spending my life savings on armor, bullets and some uzi-type weapon for drive by's. I got into a slow moving difficult-to-destroy vehicle and drove down the side walk killing pedestrians at the highly-civilians-populated (college/university area?) while mowing down civilians on the other side of the side walk with my gun. After I got out of range of the school, I would turn around and drive on the other side of the side walk and repeat.
When my car finally got demolished by cops, I'd steal one of their cars and later a Swat car which took a lot of damage.
I did this about until about 6 vehicles got destroyed, I took off on a high speed chase and found some windows I could drive into with a ladder going to a safe 2nd tier window where I was able to snipe people and police over and over again.
After about 5 minutes of this, police finally ran up the stairs where I jumped down to escape only to get surrounded from all sides. I started chucking grenades randomly and wildly until I was arrested. Where a grenade then blew up and killed all the police around me. Police cars driving to the scene ran over grenades, caught fire, careened into crowds of people and blew up killing even more people beyond my control three times despite being already arrested. Then I avoided jail time by some technicality.
It was the most fun I've ever had in a game.

tldr: Fun happened in GTA.

Since I didn't die, my best unexpected death happens to be when I used a fast-travel in Fallout New Vegas where I spawned 20 stories in the air and fell to my death.

I played Magicka with some friends, and one of them planted some 'Healing Mines' is the best way I can describe them. I stepped on one, and it made me fly off the map.

That game is glitchy, but SOOOO much fun.

This isn't staged, this genuinely happened. I had "auto-record on death" (or whatever it is) switched on, so when I saw "video saved!" as I awoke from hospital, I knew.. this was too stupid a death not to try and milk a laugh from.

In Crysis I love Maximum strength-ing a soldier in the face, then to my dispair realize that the weapon they had attached to their back fell onto me, killing me... Crysis has kinda strange physics kills
I don't know how many times I've died because a tree branch landed near me haha

I remember I was playing Halo 2 on Xbox Live. It was the laggiest game ever. I was fighting someone and threw a plasma grenade at them. The animation happened and no grenade flew. I was like "ok whatever". I died. I was crossing the same spot after killing a guy about a minute later. Then I exploded and I heard the announcer say "suicide". I was confused for a good minute until I figured out the grenade finally was thrown into my back.

COD:BLOPS earlier today, on Nuketown CTF. Spawn upstairs, decide to dive headfirst out the window. I land perfectly on my belly, not two feet from an enemy who was running that direction. I let loose a burst from my rifle, but he'd seen me and had been tracking me all the way down.

In all actuality though, the funniest deaths I've ever had were in GTA4 after I beat it and started using cheats. Nothing quite like getting into a firefight on Liberty Island- no infantry ever spawns, and the boats are too far to do anything. The only danger is helicopters- and with the weapons cheat it was open season. I was just about to spawn my own and leave, so I fired off a last shot at an enemy chopper. Beautiful hit- caught it right in the tail. This pilot, though, was smarter. The chopper spun towards me and started flying forwards. The chopper was going to hit beside me, though, so I wasn't worried about it. It wasn't until a moment before impact that I noticed the problem- the main rotor was still spinning. I died from a rotor to the head moments before the chopper slammed to the deck and exploded. I actually managed to get video of it, and if you guys are interested I could get it out.

EDIT: WAIT HOLD UP forgot a good one. (I'll try to keep it short)
Bad Company 2, Valapariso map, last point, attacking. Blackhawk spawns, and I'm an alright pilot, so I grab it and start into the enemy base. They've got a guy on the MG and I take some pretty serious flak before I get close, and I can't get away now. My only chance to salvage this is to ditch the aircraft on somebody. The busy middle section is my target. I cut the throttle and swoop in, jumping out just too late. I die from fall injuries. However, the chopper collides with someone and kills them. The best part of all this was how the game recorded the death. Not death by physics, not roadkill but Death. The game had no damn clue what I had just done. I assume dropping helicopters on people wasn't part of testing.

Good ones are those which are accidentally self inflicted. Like when that grenade you just tossed bounces right back to you.

But I think the best are just blind amazing coincidences. Like when I was playing Battlefield 2 and a crashing airplane just happened to land on me, or when I shot a tank shell up into the sky for giggles and ended up accidentally taking out an enemy jet that just happened to fly right into it... Awesome.
