I thought Samara seemed like a good team leader choice as well, but if you have her lead the secondary team the first time your group splits up, your person in the tubes will die. I'm not entirely certain of the logic of this, but even Tali or Legion will die at this point if Samara leads the second team.
How would she? She's a Justicar. They spend hundreds of years fighting criminals on their own. The only one I thought capable of leading a team back then was the wicked, awesome, cool Garrus.

As for Mordin, he's obviously pathetic in battle, so I used the escort task as a chance to get him away from the worst fighting.
No way! Get his Incinerate to level four, it burns through anything.

No one died for me, mainly thanks to the handy little cheat sheet Game Informer printed. I know, it's cheating, but I would have felt horrible if I lost one of my crew, especially to those dirty drones.

Didn't lose a single team member or crew member. Did two playthroughs and both times I came out victorious.

On my one playthrough, I lost Jack, near the very end at the "hold the line" part. I sided with Miranda during their argument, and never did get Jack's loyalty back. Can't say I was too upset...

On a side note, has anyone else seen the neat article about the Suicide Mission in Game Informer a couple months ago? It has a really thorough breakdown of all the variables and results. Quite interesting.

Man I love this game. First playthrough I lost no squatmates and no crew members, so I was pretty happy.

HIGHLIGHT OF THE GAME: Punching the news reporter in the face. I honestly didn't mean to, I saw the renegade option and thought I'd push the camera over, but no. BAM! Left hook to her cheek. Fuck yeah I felt badass.

LOWPOINT OF THE GAME: Staying faithful to Liara. I had Miranda and Yeoman Chambers throwing themselves at me. But I stayed faithful, and as a result, lost out on a 20g achievement. Fuck that! I didn't realise I'd be punished for staying faithful! Go back in time so I can get with Miranda!

Also, I chose to blow that Reaper ship sky high as a "fuck you" to Cerebus, but I'm not so sure it was a good idea. But the fact that I'll have to live with those consequences, good or not, in ME3 actually makes me look forward to it more.

Oh, and also, all of the crew except Dr. Chakwas died for me, too. I think I did a few side missions after the crew was abducted, so they were rendered into genetic mush before I could save them.

But maybe you'll be extra-rewarded in ME3 for staying faithful! Not that Kaiden/Ashley deserve it with the way they chew you out after saving the colony...

My first play through, I lost Jacob in the ventilation shaft, Samara as the second team leader, and half my crew to those tubes.

I only shacked up with Liara because I loathed Ashley, and could not get with Tali.

It's actually pretty strict on the survivability factors.

Specialist choices: (if you deviate from them, they will die or cause another member to die.)

Team Leader 1 - Garrus or Miranda
Tech Specialist - Legion or Tali
Biotic bubble - Samara or Jack
Crew Escort - As far as I know, any. Grunt and Zaeed are almost certain bets though.
Team Leader 2 - Garrus or Miranda
Hold the Line - Do NOT leave Mordin to hold the line, take him with you for the final boss, or send him back with the crew as an escort. He has a super high chance to die here.

Additional factors :

Loyalty - If you dont have a member loyal, you run a serious risk of them dying.
Upgrades - Armour, Shields, Guns. Everything else is optional.
The Crew - If you take the time to talk to your squad mates after every mission (including random encounters on planets), you should have everyone's loyalty mission done by the time you jump the Omega 4 relay comes. If you don't go through the relay IMMEDIATELY, the crew dies - except the doctor. And if your laziness is responsible for Kelly's death, you're a horrible person.

Achieve all of this, and you'll get the perfect ending. On my first playthrough I didn't lose anyone.

Hell, why stay faithful? If I romanced Kaidan/Ashley in the first game I would've just cheated on them in ME2 seeing as they didn't want to have anything to do with me anymore. As for Liara, well she lost whatever made me like her in the first game and has turned into a psycho *****.

The first time: I lost Garrus (because I misunderstood the vent mission and thought that they's have to do some independent fighting in addition to tech), then Mordin (I threw him away so that he'd say "I held the line"...and he did), and finally Miranda died because she wasn't loyal. I'm gonna redo this Shephard anyway.

For the second Shephard: I threw Garrus away in the same way as the first for character reasons, and Jack died because she wasn't loyal.

Felt pretty bad when he died, knowing that I practically sacrificed him so that I'd have a better change against the Reapers in ME3 (don't know if it will matter whether Legion survived or not, but it still seems logical).
(Could a character that isn't the best choice still survive if you opened those vents fast enough? Shepard was a sniper so Thane took some damage because I wasn't moving so fast.)

I also lost all the Cerberus crewmen except the doc... Felt pretty bad about that too, though I did not realize it actually mattered how fast you entered Omega 4. Darn games suddenly starting to enforce common sense.

Didn't lose anyone else, even though Miranda wasn't loyal (I did do my best to keep her out of harms way though).

The second playthrough (with female Shepard) I saved everyone... Well, had to cheat, since Samara got shot, which was kind of cheating, but I knowingly made a few less than ideal choices, including putting her in charge of the other team the second time you split up (it's stll pretty weird that's supposed to be bad choice, since the game pretty much tells you that you made a perfect choice if put her in charge the first time your crew spilts up), just for the sake for being different from the previous save, so I gave myself the permission to do that.

I had everyone but one character loyal both playthroughs, which must be why I had so little losses. I might have actually had also maxed out levels and every character with the best "specialist" rank, if those matter.
About the waiting too long before going through the relay... I think that timer only starts after you do the IFF mission. When I played through the first time, I had finished every side mission and loyalty mission before doing it, since I didn't know what would happen next and if I would have a chance to do them later. So that meant the only other thing left was Legion's mission. So if you complete everything before the IFF mission, you will be able to save your crew since there is nothing left to do.

I didn't lose anybody in my first playthrough, because I made good choices about who should do what.

Grunt(I almost typed Wrex) to escort the crew back

Legion and Jack to help me finish off the final boss.

You didn't look through the search bar very hard. This thread has been done countless times. I lost nobody. Mordin died at the end for you because you didn't leave enough heavy hitters like Grunt to hold the line while you fought the reaper. Mordin has the lowest defense stat (hidden stat that only applies at that time). If the defense stat of everyone left isn't high enough, some characters die in order of non loyal members first, then lowest defense to highest. The lower it is, the more characters die here, and since Mordin's is the lowest, he dies first.

I lost Garrus because I let Miranda do the bubble, and I lost Grunt right afterwards because I let him lead the 2nd squad.

First play through, no one died. Including the crew. All loyal, full paragon.

No one left behind. No fatalities.

I sent Tali into the vents, because she had the most experiance with exotic technoligy and she had a hazmat suit (Mordin and Legion also work apparently). Garrus lead my alt teams both times, because he had the most military experiance, I knew I could count on him, and he was the goddamned batman in space before I recruited him.

I used Jack as my biotic, since they made so much shit about how she'd been worked on since birth to be this super powerful biotic. I trusted Jacob and knew he was the most flexible, so I had him escort the crew back to the ship. Plus I never used him.

I had Mordin and Thane in my squad the whole mission, because Mordins fire ability was invaluable for knocking out groups of collectors, and Thane had the Warp ability to bust collector biotic shields and his shredder ammo and weapon skills made him generally useful in combat.

I finished off the Reaper with that BFG Nuke launcher, btw.

End of the game, only upgrades I hadn't gotten were a few biotic recharge upgrades and Legion's sniper rifle.

PS: it turns out it was a good thing I kept Mordin and Thane with me, since they have lower 'defense stats' and would die easily. I left all the real heavy hitters like Boba Fett - I mean, whats his face - and Grunt and Garrus back to hold the line.
