Can't promise but the only way I can think of...
- Upgrade ship shields, armour, cannon
- Do all loyalty missions (ignore Miranda's or complete it, probably doesn't matter but everyone elses for sure)
- You should make it to the station with no losses. Then when picking people for jobs, choose Miranda to do biotics section. Only Jack and Samara can do that part and stay alive. Use Tali/Legion for first bit and Garrus both times as 2nd squad lead, send Mordin back with crew.

I'm almost positive that would do it :)

I haven't let anyone die in any of my playthroughs, mostly because I'm a total completionist.

Squad setup for my first playthrough:

Technical Specialist: Tali. I wanted to slap Jacob when he was like, "I'm perfect for this!" You have all the technical skills of my dog. No.

Leaders 1 + 2: Garrus. Didn't even need to think about it. I freaked out a bit when he looked like he was going to die after he got shot. Don't scare me like that! I almost hyperventilated!

Biotic: Samara. I didn't need to think about this one either, but I did want to slap Miranda. Partially because she was stupid enough to think her genes made her perfect for everything, and partially because 'everything' included keeping my team safe from the evil bugs. Newsflash, Samara (and Jack, but I was afraid she'd drop the shield in order to kill things on my first playthrough)are better than you are at it. DEAL WITH IT.

Escort: I think I picked Thane because he works well alone and I figured he'd use his assassin mojo to kill everything. Mordin would have been better, but oh well.

Final Team: Garrus and Mordin. I didn't realize then that picking Mordin probably saved his life, and that Garrus probably should have stayed to keep the squishies (Tali, Legion...) safe. I pretty much just lucked out on this one; I was focused on blowing crap up instead of tactics at this point. Go team?

I had all the upgrades and finished all loyalty missions (except Legion's, but I finished his loyalty mission before my crew got taken) before I even messed with the derelict reaper; I figured it would start something important, like endgame. Bioware, I love you, but you're predictable.

I immediately grabbed my superhero cape and rushed off to save my crew first chance I got, because I wanted my snarky engineers and drunken doctor back. My thoughts on Kelly were, "Oh. Hm. I guess we can save her and the other crew members too, while we're there. Engineers and doctor first, though."

Yeeeeah, I'm a terrible person.

On my first playthrough no one on the real playthrough, I did manage to get 4 killed on purpose just to see how many could die just based on things I decided to do.

Not one. Although I did have to load to do Legion's mission before using the relay (because I was doing something else when you-know-what happened).

I managed to lose Grunt (i cried for him) and Tali (i jumped for joy). Clearly you are not supposed to get Grunt to lead the distraction team...

...died during the suicide mission?

im asking this because i just beat the game and lost 5 squad-mates(Legion, Thane, Grunt, Jack, and Mordin) and i was wondering if anyone else had a similar experience.
How did you manage to lose five?

Missing ship upgrades?
Lacking loyalties?

I had everyone survive the suicide mission, I had all the upgrades I could get and every squad member's loyalty except for Zaed, which I missed because I didn't kill Vigo.

You can get his loyalty anyways, with paragon speech options, convince him to fight in a team, leave his hatresd behind him for a while bla bla bla...

OT: Only once did I loose a team member, namely Thane who i sent into the vents as the specialist. On my second play through I realised my mistake and everyone survived.

I started losing people on my first playthrough... Mordin, then Zaeed... and at first I thought it adds to the story with a death or two... then Tali got taken by the seeker swarms... damn those screams gave me a chill... So then I decided to do it without losing anyone. My latest one (my main one) I lost Jack and Zaeed. Good ridence to Jack, but Zaeed was cool

Only Tali died, but that was pretty heartbreaking. While not my favourite, I'd grown very fond of her.

First playthrough I lost Mordin because for some reason I thought he could function as a tech expert. No, I don't know how I got that idea. He's the only one though.

First time Mordin, I didn't know about the HTL score. I was essentially recreating my ME1 ending where I brought Garrus and Ashley, so I brought Garrus and Miranda. Fixed that, brought Mordin, everyone got to live. And such has happened on every subsequent playthrough.

First playthrough was only Miranda.

I found it hilarious because nobody else could manage to off her. I cried tears of joy because I hated her so much.

I replayed the suicide mission until Jack and Zaheed died. I didn't like them and I don't want them in my ME3

I think not doing her Loyalty Mission is enough for her to die.

OT: No one. I wouldn't let them die.

Fucked up second time though and lost Zaeed and Katsumi, was gutted.

You can have no one die, or have everyone except Joker die.

I just completed Mass Effect 2 again yesterday and Tali died because she wasn't loyal (I wanted to try releasing her father's data to the Conclave).
