Arms clipping and bending in unbelievable ways when a player rag dolls upon death. This has always bugged me in VALVe games, ruining the sadness and disappointment that goes along with watching a Rebel die or being unable to save Zoey from a tank.

Guns clipping through walls in multiplayer from the view of another player (looking at you Call of Duty)

Clipping issues. I'm looking at you capes.

When the combat shows me as having stabbed the enemy straight through their chest yet they only take 5% of their HP as damage and continue on as though nothing had happened.

"Hi, I'm Mr. Exposition. Please don't think too hard about what I have to say".

When games are open world but don't have a good 'hardcore mode' I love having to sleep/eat/drink in big games like that, but only FalloutNV has really implemented it(and not really well either, I mean sleeping every 10 or so days?)

Poor animation during important story scenes, totally breaks the sense of immersion for me.

That scene made my laugh my freaking head off, not get into the mood of being a powerful beast.

Unbindable buttons especially in console games, where an (digital) action needing split second fast reactions like block is mapped to the trigger button which have a rather large deadzone. I'm sure everyone here with a 360 has died more than a few times because they didn't have time to press the trigger button in 1cm

Also I have an arcade stick why don't the games or the console itself let me bind myy keys so I can use it with all digital 2D games. ggrr

When items that are supposed to be on something end up under or inside of it. Like the items at the Zenithar shrine east of Gjukar's monument in Skyrim. 90% of the time the amulet of Zenithar (the whole damn reason I go there early in each game) is usually sticking half way out of the side of the altar on the ground beside it and you have to move the target over it to actually see its there.

Recently the speech skillbook and 2 silver ingots are stuck inside the altar with no way of getting them so you need to come back later and hope they spawned in collectable space -_-

I also hate any game where the enemies don't play by the same rules as you do, and its not intentional.

Like being able to pass through/over an object you can't, make a shot when they have no line of sight at all, are using the crappiest gear in the game yet can one shot you and refuse to die etc.

Also anytime a game has debuffs that almost never work on enemies but almost always work on you.
