The first time you see a zombie in Resident Evil 1. I was like 8 when I saw that and it scared me so much I've never played a Resident Evil game since.

I was playing Daggerfall, and I was walking around in the first dungeon. I rested a bit to get my health back up to maximum, and once rested, I saw a door open, and heard a really scary and loud shriek, or scream, or whatever.

Anyways, I shut the game off.

Made sure the next time I began playing, I would know the exact way to the exit.

The Dahaka in Prince of Persia: Warrior Within. Thing scared the shit outta me.
Fucking up a wall run and giving him the chance to catch up was bloody nerve wrenching.

This was actually a bug that I encountered the first time I played Mega Man Legends 2. In the desert dungeon, there is a corridor with two ornately carved panels in one wall, at some distance from each other. The patterns in the panels are cut all the way through, allowing you to see into the enormous, dark room beyond (but keeping you from entering it.) There is a huge, scary, T-Rex - like boss in this room, which you will eventually have to fight.

The point of the panels is to give you some idea of what to expect later, as the T-Rex boss will ram its head through each panel with a terrifying roar just as you walk past it. A basic, cheap scare tactic. However, as I said, this sequence bugged out a bit the first time I played the game - and in a way that actually made it work a lot BETTER!

What happened was, I entered the corridor, spotted the first panel, got curious and decided to peek inside. Nothing happened, and I could see nothing but darkness. Eventually, I decided to move on. When I got to the second panel, I decided to have another look, but was interrupted by a sudden crash/roar coming from elsewhere in the corridor. I made my way back towards the first panel to investigate - and to my surprise, something had destroyed it! I barely had time to register this fact when I heard that same noise again. Sure enough - on my return to the second panel, I found that it was now reduced to rubble like the first. Curious as to what might be causing this, I returned towards the first panel to have another look into that cavernous room beyond. (I don't remember why I didn't just do this at the second panel instead...)

And as I approach the first panel, Mr. T-Rex decides to poke his head through the broken frame and treat me to another spine-chilling roar. THIS is how I learned what would be waiting for me further down the dungeon! (And what had happened to the panels.)

I decided to close my investigation at this point, and get on with exploring the dungeon. But the scariest part of my experiences in that corridor, still remained:

In order to move on, I would HAVE to walk RIGHT past that second panel!

Never had much scare me in games.
But one game always managed to make me look over my shoulder whenever I was alone. Can't quite remember the game, but it was on the GameCube. Whenever your character's sanity started to drop too much, or something that was absolutely not right, the game would do something to you. The gamer.
Something I remember is turning the screen black with a green "VIDEO" in the top right corner, to make it seem the video cut off. Then you'd hear this bloodcurdling "AAAUUUUGGGHHHhhhh....!" from your character. After a bit, the video would come back, and you'd be exactly where you were.
Maybe a few bloodstains on the wall, or a corpse that wasn't there before, but just fine otherwise.

The ladder moment...F.E.A.R. 1
Both of these! I was really hoping someone would mention that ladder portion of F.E.A.R.! I think it is one of the single biggest, "HOLYSHIT!" moments in the entire game.

Yes, I know there is no space in my exclamation. That's how I said it when I first experienced the dreaded ladder of death!

I would also add the bathroom mirror in Doom 3 - The one in the starting area. That thing scared the piss out of me!

Was that the one were one minute you could be walking along then the next you'd be in a desert and the skin would have peeled from your bones.

System Shock 2.

No contest. Bioshock was somewhat scary, sure. But the same reasons that people threw in System Shock 2's face (like degradable firearms) were the EXACT thing that made it so damn scary ON TOP of the insanely intense music and atmosphere.

*Scary music plays as you hear mutants moaning towards you and you need to fire a level 1 degraded pistol at them. IT BREAKS! Shit...time to go melee and dodge them as best you can!*

Nothing like that feeling of being an adrenaline-pumped underdog and realizing that you just felt like the king of the universe not for beating a final boss, but because you managed to melee two hybrid mutants with your wrench. :)

The haunted house level in Super Mario 64, where the piano comes to life and tries to eat you. When I first played that section I was still pretty young and new to videogames, so I flipped right the fuck out. My dad had to rush in and figure out how to exit the level because I was to distraught to do it myself.

So many. In Silent Hill 1 in the alternate school when the locker that's dripping blood is making noises as if something was strugling inside, and it turned that there was
nothing inside

Bioshock.

There was one point where I entered this flooded (like, knee height, not underwater) basement, it was completely empty, and I ran to the far corner to grab a plasmid, when I turned around, the room was filled with creepy ass mannequin-like statues of people, scared the fuck out me!

Of course as soon as I turned my back to any of them they attacked.
Ugh the dental office? Scared the crap out of me too.

Although while a lot of games are decent at the "jump out and boo" scary, I think Silent Hill 2 has to take the cake for "spooking out". Because you are alone. So very alone. Like, not "oh there's a monster around the corner" alone, but just you. Especially in the beginning when you're walking through the streets with nothing but a large stick and you run into a Straitjacket.... URGRAH!

In F.E.A.R. 1 there were these stairs you walk down and on the right is a alma scene and im like "oh its just alma again step back then go on again" but then i turn to the right and that one dude who your hunting is there and i was like " Oh fuck bam bam bam grenade" yeah that one got me scared for a couple of secs.

And as others have mentioned the ladder scene well it didnt get me..but that was cuz i saw it out of a glass of soda i think.

That part in the first stalker when you are under the military base and that controller comes out of the darkness and makes you zoom into his face. It made me cry.

Actually it was Poseidon Plaza, but shit, you just reminded me of the Dental Office, that was scarier
