Pit Boss from Portal 2. Not only is it hilarious that you get an achievement for jumping into a bottomless pit of death, but everything that Wheatley says if you wait several minutes before jumping is priceless.

BarbeQueQ from TF2, which in essence rewards you for causing another player to ragequit when you dominate them as a Pyro.

When you fire about 5 playthoughs worth of shots in Borderland you get the reward/achievement thing "I fired every bullet ever" that always seemed funny to me.
I got that in one playthrough. Your not suppose to let go of the trigger button right?

As for me, I'm not really sure about any in particular...

One of the most hilarious moments for me personally was a moment so bad I had to stop playing for a good 10 minutes because I was laughing too hard.

I was playing a demo of Ninja Blade. The game's boss battle was frustratingly long and boring having to repeat the same attack pattern a seemingly infinite number of times. Along with the boring fight were a series of quick time events to proceed the ridiculously long boss fight to the next stage of the boss fight. Quick time events aren't really that hard so I was just breezing through. It wasn't until the end when I was really starting to phone it in when it happened.

I missed a quick time event.

It wasn't that exact scene, but it made me burst out in laughter. Just when I thought a game couldn't possibly get any worse I witnessed that laughable excuse of a punishment for missing a quick time event.

I put down the controller and proceeded to laugh so hard I almost coughed up my lungs.

Not scripted nor an achievement, but i had a hilarious moment in Gears of War 3's four player co-op on insane. We were playing Act 2, and there were about 4 savage drones firing at us from their little cubby hole. No-one could really do enough damage to them and we were kind of pinned down. I snuck around and ran up a bit closer to the drones who were busy engaging my buddies, i whipped out a frag grenade, lobbed it and took cover. One of the drones just sort of turned slightly, noticed it and went "...GRENADE!!!" followed by the loudest explosion and sound of gibbed chunks of meat splattering everywhere. I got a quad kill for that moment of glory and everyone on the mic had a good laugh. It's moments like that that just make you go "Oh, SNAP!"

Two opposing sides try to convince you which side to take (The first two choices are who to believe and side with).

The 3rd option... Fuck this, I'm leaving!

And he does, the world is infested with the undead but he doesn't care and moves on with his life.

Consolation Prize in TF2. I laughed my ass off when I got this achievement. I am horrible in TF2 and the game recognizes it.
I love that achievement, awarding you for dying fifty times by the Sniper's natural enemy.

Not an achievement, but I was running around in Assassin's Creed 1, just looking for someone to fight. Then I was running to a corner and rounded it while jumping off of a small ledge. It was timed perfectly so that I jumped right into a guard and accidentally teabagged him in mid-air.

I've got a few funny moments, but this is one that I happen to be thinking of right now.

A few years back, I was playing Civilization 2: Test of Time. It had a Fantasy style mode in it. So I'm playing as the merfolk, and have a fine-and-peachy alliance with the humans next door. Then...I finally got DRAGONS. So I built a few, and then decided to be nice to the humans and give them one.

Their response: "We have no need of your USELESS Merfolk weaponry!" ...they said that about a freakin DRAGON (that I was offering FOR FREE). Then, they almost immediately DEMANDED I fork over 500 gold in order to keep this alliance intact. I said hell no. They then dropped the alliance and declared war on me. I had to sit back and take that in for a second before laughing at their sheer stupidity.

What did I do? I sicced the dragon I'd been about to offer them onto their capital city. Things burned. They asked for a cease fire. I declined (With a grin wider than the moon). I continued the burning. FINALLY, they said "STOP! WE OFFER YOU 500 GOLD! JUST PLEASE SEND THAT DRAGON AWAY!". ...The irony was just too delicious. I took them up on their offer. And then demanded tribute for my patience, causing them to cough up even more money at me.

...They learned their lesson and toed the line from then on.

A side quest from The Witcher 2 that had me in tears laughing. One of the funniest quests I have ever seen in any game.

You have to help a bunch of drunk soldiers find their even more drunk friend who has gone missing. After you find him, you have to lead him back to the camp to his comrads....and lead them back to the pub XD

Seriously, the whole thing was just genius.

Apache Assault: It's basically a helicopter combat game. In the beginning, your base is being attacked and you have to get to your helicopter. With this comes the many "get to the choppa" jokes that go with it... and the "Got to the Chopper" achievement.

My favourite was Rasputin for Team Fortress 2. It requires you, as a Heavy, to take bullets, get burned, get beaten by something, take damage from explosions and maybe have some engineer buildings harm you; not sure but it's something like that. Now, when I read the description I laughed for a good minute as I know the story of real life Rasputin's death, which is just about as hilarious.
This.

I have had some AMAZING failures in games...

In Ace Combat ZERO I managed to crash on takeoff. See, what'd happened is I found a glitch, where I had somehow turned in just the right way where my plane could angle itself upwards while still on the ground. I kept angling up and up and up and BOOM.

Then in Gundam Vs. Zeta Gundam I was playing as one of the infinitely flying fighters in the game. Now, don't ask me HOW I managed this, but I was fighting one of the boss mechs, which was a giant flying gun fortress... Thing. That was completely useless. So I flew up to it and pushed the missile button. And missed. At point blank range.

This one isn't really a fail... On my part.

So my older brother always played this game called Armored Core: Silent Line. Now, I only recently got into it and so I sucked at building a mech. But bro, being cool like that, decided to help me make one, and later challenged me to a match, like he always did years back, only now I'm competent at games...

Turns out the thing we built was a fucking monster. It was only armed with a laser blade and a machine gun. Thing is, it was also strapped down with so many fucking boosters it was almost impossible to hit. We re-named it the Berkut, otherwise known as the golden eagle (I think), which is known for killing FUCKING HONEY BADGERS [http://www.youtube.com/watch?v=c81bcjyfn6U] FOR FUN (... I think.).

... Now if only my fucking PS2 wasn't broken, I could play these games more.

Aaaaalso in shimmering Isles, theres a quest to help a guy kill himself, but he asks that he doesnt see it comming. You wait until late at night, hes up on a precarious ledge overlooking the town. musing about how the view from just this spot is so perfect, and serene, that it reminds him that his miserable life isnt so bad after all. At which point you yell SURPRISE! "Wait, WHAT?!" And he goes for a short flight.

If you havent played Shimmering isles, it's hilarious and enjoyable. ^_^
That quest was by far my favorite quest in the entire game. I laughed so hard.

My favorite moments are from Timesplitters: Future Perfect. The drunk guard/henchman/random guy that appears in every level always makes me laugh. I always stand there and listen to their entire rant. I'm glad the developers put that in there

On TF2 when as a Spy I used the Dead Ringer to fool a Scout, he then proceeded to taunt over my "dead body" I went right up behind him, uncloaked and backstabbed him midtaunt

Must have been a real "oh shit" moment for him though, seeing the Spy you just "killed" slowly reappear behind your cocky defenceless ass

Building on from this,

Rasputin -- in a single life, get shot, burned, bludgeoned, and receive explosive damage

Portal 2 "good listener" achievement. I trusted Glados and she tricked me :(
She tricked me too...I trusted her and she tricked me.

Recently I bought Syphon Filter for the PSX. The main reason for that being the taser. It has an almost unlimited range and if you hold it on an enemy for long enough they burst into flames. Freaking hilarious!

F1 2011, I've literally just started playing (it dropped through the letterbox about an hour ago!) and the handling is different from last years. First run around the Albert Park circuit and I spear it off onto the grass. My reward? Being asked by the game "Have you considered Dirt 3?" Cheeky bastards...
