Quake 4 had a bunch of achievements for playing the multiplayer. I tried to see if it was any good, long story short, it wasn't. No one in their right mind would play that shitty tacked on multiplayer for as long as it would take to get the achievements.

Vasebreaker in Plants vs Zombies. 15 levels of completely random crap. Easily the most frustrating achievement I've ever attempted and ironically enough, from a game aimed at middle aged mothers.

(yes I know, casual game but I didn't buy it so I don't have to hate myself as much as if I had)

Left 4 dead, killing a tank single handedly

Whenever I tried to do it, some retard would fire and ruin it

Probably the most annoying ones I'm currently working on are the "50 speech successes", "50 computers hacked", and "Nuka Cola Challenge" ones in Fallout 3.

They're the only 3 I have left to get and are annoying as HELL. There's only about 100 computers in the game, and they're all spread out in different areas, so that as soon as I tack one more on, I need to move to the next zone just to get a DIFFERENT one.

The Nuka Cola challenge is painful mostly because of how scarce these things are. Every... say... 5th vending machine has ONE nuka cola in it, and I need to find 30... OI.

Oddly enough, I found the lockpick one surprisingly easy mostly due to the fact that almost EVERY door is locked, and with a lockpick skill of 75 you can get almost every single one.

So, yeah... They're doable but more annoying than some of the dailies in the World of Warcraft universe.

thats the next one im after, its gonna be hard
That one was easy, but yet again I used a glitch to kill it.
How so? It's so annoying!
It was on the hospital stage for No Mercy and the Tank spawned by the safe room my team mates died easily, but being the coward I am ran to the safe-room I closed the door. This may sound pretty dumb because the Tank can just break the door, but there is a glitch where if you run back and forth into the door. The Tank is unable to break the door open.

Flatland Flyer in Wipeout HD, Professional in Killzone 2, most of the only-ranked matched multiplayer achievements in Halo 3. Also pretty much all the platinum trophies... never gonna get those. lol

All achievements are annoying for one and only one reason: the single-handedly kill the immersion of a single-player game. I don't have a problem with achievements in a multiplayer game, but if you're trying to get through the single-player campaign, the last thing that I want to see is a little message box ANYWHERE on the screen telling me that I've unlocked some bloody stupid achievement that I never really cared about to begin with.
You can turn the achievement notification off you know...
Really? I have yet to play a game that has allowed me to turn those messages off. But then, that could be largely due to the fact that I am a PC gamer first and foremost. And what few PC games that do have achievements want to make sure you know they exist.

Dead risings achievements were fecking infuriating, as if staying indoors and outdoors for 24 hours each wasn't irritating enough you have to kill about 53,324 zombies, this was later done again in Left 4 dead but to a less annouing effect.
