Anything with a Fuel Rod Cannon in Halo: Reach. I'll also go ahead and say the flying assault base thing in FF13 (took me about half an hour to kill 1 boss).
Could not concur more.
That being said, Grunt heroes in Halo: Reach Gruntapocalypse.
I don't know if you're familiar with the gametype, but the only purpose a Grunt hero's presence serves in the gametype would appear to be ruining multikills by taking two shots to the head to kill.

Halo Grunts: stupid and pointless

On Legendary they're actually a pain in the ass.
Not totally inclined to agree.
They're absolute fodder on all difficulties, in every Halo game I've played, but they are just freaking annoying with their plasma grenade spam, constant use of the pistol EMP and their fuel rod cannon spamming.
That said, I also hate bears and cougars in Red Dead Redemption. More predominantly bears because they are more common.
The bloody things sneak up on you in the woods, charge at you unprovoked, one hit your horse, kill you in two hits send you flying ragdoll to the wall whenever they hit you.
And once you get fighting one, suddenly all of its buddies appear within like a minute, and in groups they're able to attack you again before you have the chance to pick yourself up from the first attack. So basically they suck.
Cougars are much the same but faster.

Oh, and a special place on the list? Spies. god damn fucking spies. And I don't care what you say about them being players, they are STILL ENEMIES
But... They were never on your side.

Or those suicide grunts. They're so tiny you can't help but fail to notice one, and that's all it takes...
Even worse is when you sneak up on them, make it within a metre of them and watch them spin around in their panic just before you melee them and reach
straight into their pockets.
And you explode instantly.
A PLASMA GRENADE DOESN'T EVEN TAKE THAT TIME TO ARM.
