Okami. Not because I think it's a bad loading screen, but it is SO ANNOYING trying to get the Demon Fangs.
But it's so rewarding when you do get them.

OT: I haven't really encountered loading screens as bad as the ones mentioned above so the worst would probably be uncharted 2 or the first LBP as they could sometimes go on forever.

I don't like the ones in Fallout: NV. I don't know why, they just bug me.
Probably something to do with them taking up 60-70% of your playtime.

Metroid Prime 3 was pretty horrible. It only started loading the next room once you told it to open the door, so you'd often have to wait, glaring at the lifeless panel for about 30 seconds.

It was bearable though, and the rest of the game is great.

Each and every loading screen that doesn't have a progress bar. PC games, being what they are these days, are liable to crashing. It's immensely entertaining to stare at a vague particle effect, or even worse, a black screen with "LOADING" written on it, only to realize that nothing has been going on for the past 8 minutes.

I don't know about worst, but I remember a long time ago in PS1 era that there was a game for it where the loading screen let you play FRIGGIN PONG.
PONG.

AWESOMEST SCREEN EVER.

Anyone remember what game that was?
Yeah! I was going to ask that to! all i remember about the game was that i played pong and thought it was awesome! ... they should do that with frogger and pac man and excite bike ... and astroids ...

Excessive loading screen syndrome has afflicted the Fable series since its inception. You would think this might improve after three games... good job Molyneux

Mass Effect 2, In Mass Effect the elevators and airlock and the Mako Drop kept you in the game. So who would win in fight between you and Shepard? That's good stuff. Generic loading screen 2, not so much.

The loading screen when I'd pop GTA IV into my 360, it got really annoying having to wait up to 30 seconds to get into game. Just sittin there watching the pictures of hookers and gansters kidna put me of the game before I even played it.

Also: For me, the Dragon Age loading screens never took more than several seconds (and mine isn't top of the line, its just a fairly decent laptop), so your computer is probably the thing at fault there.
There was a problem in the original version of the game - it got so bad that I was sitting in load screens for 5 minutes at a time at one point, although it did get patched.

So yeah, they get my vote too.

PONG.

I played that game, but can't remember which one...

There was also a fighting game that had those little slide-puzzles (where you have to slide the tiles and rearrange them into an image) during loading, it would even let you finish the puzzle before starting the round if you wanted to.

If you were playing with a friend, you could compete against them in assembling your puzzle. The first (and only) multiplayer loading screen ever.
Hrmm it seems many people have played this but no one knows which game lol. Anyhow, more games should do this. Coding 2 paddles and a ball for a loading screen can't be that difficult. Heck I'd take a text based adventure over "Now loading" anyday.

dead rising, because it comes so damn often and it's so damn long. i mean, i'm watching a cut scene right, and it has to load 2 more times as it switches point of view. the cut scenes in metal gear solid 2 are way longer than these and they don't nearly as long to load.

The Mass Effect 1 elevator sections were so needlessly loooooong. Same with the New Vegas loading screens, as fancy as they were, by the end of playing it 5 times, I never wanted to see it again.

Oh and the loading screens from WET drove me utterly insane.

I didn't mind the elevators. The airlock, on the other hand... It doesn't take very long to get tired of hearing "Decontamination in progress".

Mass Effect 2's. It's a set time on PC. If you want stuff to load at the actual time it should, you have to disable them.
Wait, whut? You mean deck 2 doesn't have to take hours to load?
I haven't played much of ME2 yet... are there loading screen tips that they just don't want you to miss? If not then set loading screen times are just inexcusable. Even with those tips set loading screen times are VERY DIFFICULT TO JUSTIFY. >.<

That's not as bad because at least it only says it once, instead of over and over for a couple minutes.

It's not a loading screen, but just before it:
In Baldur's Gate: "You must gather your party before venturing forth" in that annoying narrator's voice. I know there is a patch to switch that off, but if you don't, it's soooo annoying
