First of all, if you haven't learned by now to save early and often, you deserve to lose 4 hours worth of gameplay.
Yeah, I'd have thought I'd learnt by now, but some games just draw you in so well that I forget that I have to save.

Civilization 4 (and add-ons):
Start research on a long project. 3 turns (or less) before you finish, your scout moves
into a town and gives you the tech that you were researching.
Even worse, when someone else finishes that crucial Wonder you've put so much effort into one turn before you do. Not so much a problem if you do save regularly, but I point to the above statement, and also that I used to do the GoTM tournament games on CivFanatics, and you weren't allowed to save and load for any reason other than for quitting and carrying on the game later.

I'm surprised a red shell taking you out in Mario Kart five yards from the finish line hasn't shown up here yet.
My blood is boiling just reading that! So many bad memories of smug friends doing that.

Years ago I had two installs of Space Hulk on "the family PC" by accident. I used to play that game religiously and I had spent hours on this one mission where I had a lone terminator (after his other eleven bretheren had perished moments ago) fighting fiercely through the rest of the monsterously long level, bolter glowing white hot and jamming half the time from the sheer amount of lead it was spewing forth, before he finally made it to the escape ship, slamming the door shut in the face of the last ravenous genestealer. Mission complete. Basking in my triumph, I saved the game and I decided to leave it there and continue the next day.
To my horror, my father had gotten on the PC and decided that we didn't need two installations of Space Hulk and proceeded to delete one without asking me. Guess which one he deleted? I never played that game again...

Getting to the second to last opponent in Assassin's Creed, and the 360 console arbitrarily deciding to save TWO time. The second time, of course, happening as I turn off my console. File was corrupted, and had to start over =P

And Raining Blood on Expert.

Building yourself up in starcraft and feeling all good when all of a sudden your opponent comes in a wipes out all of your hard work but leaves just enough of you left to suffer until they come back to finish you off. Man I just want to whip my mouse out the window.

Final Fantasy 6 the last boss who just decides to wipe you out for no reason than it feels like it. Man every time I play it I feel like it is just luck if I happen to beat it. Most times I beat it by tossing every gil I have at him since every other attack is useless.

as already stated, the epilogue of COD4 on Veteran, but really the second half of the missions on Veteran have to be included, i think that's the longest it's ever taken me to beat a FPS.

conversely, best moment in my video-gaming life recently; getting accused of using aimbots when i counter-sniped 4 snipers in a row (2 headshots, 2 body shots) from across the level using a mp5 on COD4 multiplayer (the map was 'Bloc') that has to be annoying as hell from the other side...

Any mission in World of Warcraft that has you collect Random Drop items...
That sounds like the majority of quests in that game...and it gets worse when you've been killing dozens of monsters for one quest item and someone just walks up, kills one, and gets what they need.

the hilarious escort quests in world of warcraft where you get goodies for following a suicidal donkey around a camp of baddies. Beforementioned donkey is not concerned by the amount of health/mana you have and does not seem to realize his survival is depending on those amounts. There you are, out of mana and nearly out of health, trying to save a 'person' with the average IQ of a dead plant whilst he/she runs off into half a dozen baddies at the same time.

I generally grab those quests to watch them die and laugh, then I abandon them.

Secondly, the liquid frustration coming out my ears when I tried fable. I got this lovely skill turning me into a hulk-like figure with the ability to knock down every opponent in a single blow. However, once on the ground, I could not hit the enemies no matter what I did. I had to wait for them to get up, before I'd knock em down again. This, and the amounts of knocks it requires later on in the game to kill something gave an abundance of auto-cannibalistic frustration

That pink demon lady from Ninja Gaiden or, as I prefer to call her, the Demon ***** from Hell.
The defence part of Chernobyl in CoD 4. Still can't complete that on Hardcore; I had to change back to Normal for just that part. I'd accept any tips but I just don't think I'm good enough to keep those Zerg Rushing Russkies off my back, and sides and front for the x minutes it takes.
