Otacon.

Everyone he loves dies, and his father drowned himself while Otacon was in bed with his stepmother. Really? Death *plus* pseudo-incest? Wow.

Also, Albedo from Xenosaga. Now there's one sadistic screwed-up mofo.

After his friend becomes a demon lord he proceeds to rape Guts girlfriend in front of him while Guts arm gets eaten off his body by demons and his eye gets pieced close with this being his final sight (at least in that eye).
Not quite. His arm is bitten and held in place to keep him from moving, but it's Guts himself that chops it off to get lose. Better yet, he does it with his broken and blunt sword.
Na I know I was just being lose with the general idea of what happened if I were to be specific and accurate well I would take up this entire page with text.

Spoileeeeeerssss! You really should hide some of that but yea totally agree, guts is a pretty cool character. A friend of mine got me to watch the anime and then later he had me read the manga (both are pretty good I think). Not sure if you know but his armor is available in Dragon's Dogma. I'm pretty close to the end of the game and still using it. :)

One thing I don't get though. In the manga, when he escapes as a boy, he's chased and injured, falling off a cliff. It's cold out too I think but it kind of looks like his hand is glowing in a puddle of water. Did he have some kind of divine intervention or is he just that tough, to get up and walk away and still be able to fight the wolves?
I am fairly sure it was him being prepared to die and he sorta just fought out of instinct. Also ya I really should have put some spoiler tags up your right.

Shinji Ikari and Asuka Langley Soryu from Neon Genesis Evangelion. First of all they're two kids with serious mental issues, and I mean serious.

Shinji simply can't believe in himself, is afraid of relating with other people, has almost no courage in himself so every time he has to do something important or dangerous he becomes afraid of the bad things that can happen and when something bad does happen he always runs away from his responsibility and puts the blame on others. On top of all that, Shinji simply doesn't know his reason to live, he doesn't know what his purpose is or what he wants to do.

Asuka is just insane, she is extremely egocentric and proud of herself, so all the time she's trying to prove to the others how better she is than everyone, she always mocks other people, gets angry about almost anything, is always complaining, putting blame on others when something goes wrong and when someone IS better than her she gets insane, giving away all logic and reason to try to show that she is better than said person. But the most disturbing thing about her is that she can't accept her own feelings, possibly because of her pride. She denies the love she's developing for a character and acts very aggressive towards him, while also saying she has a crush for another guy, that's far "better" than the one she actually loves.

Another thing that's cool is that these two aren't fucked up simply because they are, they have pasts that justify their personalities.

Shinji's mother died when he was very young and his father, Gendo, couldn't take care of him by himself. He had no contact with his father and when he had, his father was always very harsh and cold about him.

Asuka's parents committed suicide and she lived for the rest of her life in a military base training to be an Evangelion pilot.
A) Shinji never blames anyone else for his issues. He spends the entire series blaming HIMSELF for things that aren't his fault and drives himself into self-loathing over things he has no control over. This is because he views himself as worthless due to the "death" of his mother and being abandoned by his father (which was ironically caused by Gendo feeling that he would only bring pain to Shinji by being close to him). This reinforces his inability to get close to others and makes him the AVPD wreck that he is at the start of the anime. More tragic is what happens to him during The End of Evangelion but because I've got no fucking clue how to spoiler shit I'll just let that lie.

B) Asuka is NOT insane and her mother, Kyoko, commited suicide after being ONLY partially absorbed into Unit 02. THis meant that when she she was left insane and replaced her daughter with a doll (going so far as to start calling it "Asuka"). As for Asuka's father (who is never named), he hooked up with the head nurse looking after Kyoko before she had killed herself. He then married her afterwards.

What's so fascinating about the two characters isn't so much that there's reasons for their personal dysfunctions (no, that's just decent character writing) but that the characters are so different despite the fact that they're so different despite the fact that their personal tragedies are si very similar.

Rorschach from Alan Moore's Watchmen. I'm struggling to write why so instead I will implore you to read the graphic novel and watch the film. He's insane, but you may find yourself rooting for him.
don't forget the comedian: also kind of nuts (in a different way), but still understandable.
Really, half of the Watchmen fit this description. Ozymandias too if you consider what he does.

I'd have to imagine that in 3 pages of responses that this has already been mentioned, but just in case it hasn't I'll go ahead and say it:

A lot of the characters in the Bioshock universe are specifically designed to be fucked in the head. We've got the mad artist turning people into his art, we've got that guy in the 2nd game that turned into a giant squid-whale thing and started becoming a homocidially insane CEO-type figure. And who could forget the psychotic plastic surgeon who decided the best way to prove that his surgery skills were art was to turn people into living Picasos? Not to mention all the random splicers jibbering insanely to themselves as they wander around Rapture.

Also, Kefka from FF6 is one of my favorite psychotically twisted characters of all time. :3

This one might be new but: Vaas from Far Cry 3 is one of the best villains I have encountered in a game.

I can't recall some older ones unfortunately.

The Ice King from Adventure Time.

Normally the show does nothing for me, but there are a few episodes which expand on his back story and turn him into a tragically twisted antagonist.

Seriously watch the episode "I remember you". Manly tears WILL be shed!

While it rests entirely on fan speculation, Sabitsuki from ".flow" fits this really well.

The widely accepted version (that I subscribe to) pins her as a young woman in the hospital who:

-has a deadly airborne disease, is dying
-bullied/killed other people who became sick in the past
-harbors complex feelings of hatred/fear towards her previous best friend
-has become cynical and hopeless, probably self-loathing
-is getting continually more insane

And considering the best ending involves getting her limbs cut off by a nurse with a chainsaw, we have the cherry on the messed-up personality sundae.

Asuka is basically Narcissism given form. She attempts to prove herself to others, pretend she is worth something and attach her identity to Evangelion's because she ultimately hates herself and is filled with self loathing just like Shinji does, they just express it in different ways. She gets attention and praise from piloting (stuff she didn't get through childhood due to the tragic death of her mother) and so she gets extremely filled with self loathing when she can't be the best because she feels like she hasn't proved herself.

Shinji on the other hand is basically the personification of Avoidant Disorder (look it up) and avoiding pain, responsibility, reality and other people due to his feelings of self loathing.

Sander Cohen from Bioshock needs his own TV show.
While Cohen was pretty messed up, I was much more affected by Dr. Steinman. His shattered perceptions of beauty and his drive to make women beautiful are so tragic and amplified by the fact that he's the first real person you meet in Rapture. That character still blows me away.

I love what Bioware does with its fucked up characters. My favorite though would have to be HK-47. One being a murderous Droid assassin (that is, a droid who is an assassin) and the other a neurotic though brilliant mind. HK-47 is just an utter delight to have in the party if only to hear the dialogue he spouts off when you're talking to NPCs. Also his attempts to cheer up the player character and his definition of 'Love' make him really quotable for me.

I was close to mentioning Mordin Solus but he's just a neurotic mess...too bad too, would have wanted to mention those seashells.

Crona own blood is his/ her weapon so it must of been tortue from him/ her. Crona had an odd relationship with his/ her mother Medusa like she wanted Crona to slay a baby dragon but Crona couldn't do it. Medusa locked Crona in a room with no light and the next day Crona was able to slay that baby dragon without a problem but with a creepy smile on Crona face. Also I still have no bloody idea if Crona is a boy or a girl! (I assume Crona is a girl since that flashback scene Crona had a dress but my mate think Crona is a boy).
I want to get LOVE carved onto every cell in my body to illustrate just how much I love Crona. S/He was the reason I watched that show to the very end, but the manga was still better cause he murders three rabbits with a STICK, then stabs the fuck out of Medusa with this insane grin on his face. His arc was really really good in both, but the manga was better.

That being said, Soul Eater really should have been a satire of anime. It flopped around in mediocrity as a serious story, with only Asura and Crona giving it any strengths.

Darth Vader. Not much else to say there.

Also most of the characters from Stephen King's Dark Tower books, reserving the most of the amazingly fucked upness for the core group of heros all who have twisted horrible pasts and struggle with their demons every day.

Going to also throw in The Phantom of the Opera and Achilles from the Iliad because literary snob is how I fucking roll.

Also Starr from Preacher...

And SPIDER JERUSALEM
