7 day survivor - Dead Rising(incredibly time consuming, mess up once, gotta start ALL OVER T_T)
Ultimate Battler - Star Ocean: TLH(even more time consuming, pain in the ass when you cant stand any of the characters except two)
Fear the Reaper - Unreal Tourney 3(...)
Enlightened Guitarist - Guitar Hero 3(my left hand is falling off...)

Hardest for L4D Safety First, no friendly even with a all bots a DAMN Louis bot somehow shot someone only ONE, Untouchables and Nothing Special. The hardest for TF2 are more about opportunity and chance

Some people commenting on little rocket man..., I carried the god damn gnome all the way to the rocket. And didn't close the door. And didn't get the achievement. And I had overwritten the save. And I had (still have to do it) to replay halfway through the game (again) to CLOSE THE TRAP.

I feel your agony, nothing is more a pain in your ass than to have to drag that piece of clay through the game twice. I wonder how many other people made that same mistake.

7 day achievment on dead rising or seriously in gears of war 1. Wasn't there also a sports game where you got an achievment for beating someone on the very top of the scoreboard? That would seem like the hardest just because you have to find one of the players, get them to accept your challenge and beat them.

you have to make it through a plane in 60seconds on veteran. Its near impossible.

Safety first is easy if you play in easy with 3 bots. Always crouch, don't shoot. I think I killed about 30 zombies in the whole run.
Untouchables can be done on easy in death toll, if you stay in either the rock or the dock. Get 1 sniper, and should be fun.
Nothing special IS hard.

One that was fairly easy, but agonizingly annoying was the little Rocket Man aheivment from Half-life 2: Episode Two, in which you have to carry a gnome through the entire game...and shove him under the steering wheel of your car and continue to reshove every five feet, if it wasn't for the whiny little ditsy sitting in the passenger seat i would've abandoned it a long time ago....Try it, see weather or not you get reduced to tears...
