Have you ever played Phantasmagoria? most of the game is live action cutscenes, one is where you character is strapped to a chair, and she shortly gets her head smashed open like a cantaloupe with a a guillotine like device, and you see every bit of it, unless you turned off the gore because you were to afraid of seeing stuff like that.

Oh man, Phantasmagoria... The game was all about disturbing cutscenes. I think this is the most disturbing, though:

In Gears of War 2, when Dom finds his wife in the caverns...I swear to God I almost cried...that was crazy awful.

That and the thing with Tai actually affected me, I was amazed.

EDIT:

Both parts of the battle with Saren in Mass Effect. In the first part, you get to kill Saren in that oh-so-special way which I totally did not see coming even though my character's dialogue alluded to it constantly; and in the second part, when his dead corpse (or rather, the mechanical implants he received from Sovereign) is reanimated and he turns into the Saren-Geth-Hopper Deluxe Death-Bringer with extra death...

also that little chat with Sovereign on Virmire.

It wasn't as horrifying as other games I've played, but it was pretty creepy.

And Space Giraffe. That... was... fucked... up...

True, those scenes were also powerful, but a bit more mentioned than the one that actually affects me, the one with Fai Dan that I linked to.

I would have to go with either the ending of House of the Dead Overkill (you mean he crawled back into his mutated mother's...you know...?) or the 'your beloved sister was just mutated into a giant insect-like goddess of doom with spraggly eyes and is intent on destroying the world, and you along with it' ending from Drakengard (there were many disturbing and fucked up things in that game, in fact, that's partially why I liked it).

Oh hell yeah I loved that game!!! The best scene is linked in a post above this one...

Also, a lot of the scenes from Saints Row 2 were pretty raw. Graveyards and Monster Truck rallies, man...

It may not be too terrible for most people, but in Saint's Row 2, when Johnny buries some guy alive in his dead girlfriend's coffin. For some reason I thought it was the most terrible thing I had ever seen. Getting buried alive is probably my biggest fear, and I couldn't help feeling awful for the guy getting buried.
