For me, definitely the bottle-to-the-face scene in Pan's Labyrinth. It's such a ruthless and fairly graphic death; that combined with the victim's innocence shook me up a bit.
I have to say, that one was quite alarming for me. I honestly didn't know that Pan's Labyrinth had an R rating the first time I saw it and up to that scene, there had been nothing graphic about that movie.

And then I watch a guy's face cave in...lovely.
Agreed, that scene REALLY caught me off guard. Now im massively into horror and gore flicks, but that scene just disturbed me for days. may have been because i was ill when i watched it (and my emotions and stuff are messed up when im really ill), but goddamn, savage stuff.

Also i gotta admit the from the HBO Rome series, there was an episode where a guy was tied up in a cloth sack and thrown in a fountain for a while, then pulled out and staked. that was pretty disturbing too.

Does anime count?

If it does, pretty much anything in Genocyber.
The detail that was put into some of the deaths was sickening, especially the ones involving children.

That or the chainsaw scene in the appropriately named Violence Jack.

Guy in some zombie movie, forget its name, land of the dead maybe, where he gets grabbed by like 30 zombies and they rip him apart and eat him... and you see it, yet he yells out 'Choke on it" to the zombies.

Fairly certain that's from Charlie Brooker's Dead Set. It's the producer who gets torn in half, if I remember correctly.

The Mist, just cause the guy killed everyone, including his son, then the military shows up. Also Dawn of the Dead where the whore-ish chick gets sawed in-half when the bus tips.

For me, and the audience in the theater that I was in, it would have to be the final scene in Seven. It was very quiet in the theater after the movie ended, just because it was something we weren't expecting.

I have two that have really gotten under my skin. The first was in final destination 2 when that kid gets crushed by a pane of glass in front of his mother, that really chrurned my insides up terribly.

The second one is more sound based. When that soldier gets his eyes pushed into the back of his head by Jim in 28 days later. The sound of a grown soldier screaming nearly made me sick.
Yep I was gonna mention the sound in that scene, but its more whenever he's repeatadly getting his head banged off the corner of the wall, it's a terrible squishig noise.

in Final Destination 2 when the lotto winner gets impaled through the eye.

That screwed me up forever. I can't even watch chicks put on eye liner anymore. I have to think of other things when putting my contacts in.
You think that one was bad, please don't watch zombie 2, there's an eye impalment scene in that which is unbelievable, the camera never looks away.

How about in Watchmen where a guy gets a cleaver to the head. Or someone has hot greasy thrown on them. These are really only disturbing to me since I can almost feel what is happening to a character.
The Punisher movie, with Thomas Jane and John Travolta, did that first, except with a large paper-cutting blade (you know, like back in art class) instead of a cleaver.

OT: Probably anything from the Saw movies, Hostel, or Halloween (like the arrow through the neck? Or bashing the sleeping bagged camper against the tree? Yeeeesh.....)
I'm pretty sure the watchmen graphic novel came before that. I've never found any death sickly, some from sin city are pretty gruesome though, like the guy with razor sharp nails, he had his limbs sawn off and then had his dog eat his half his torso then got decapitated, he never winced through the entire thing, freak.

Yes! That was awesome! I loved that scene! I have a shirt from that movie, that was so fucked up.

Also, Shine On You Crazy Diamond.

Well. I don't think media violence is disturbing, but I guess people were disturbed at the end of There Will Be Blood. I thought it was cool but the theater got quiet and weird and then people started whispering alot. As if they've never heard of the 'bad guy' getting away with it. If only people knew just how into schadenfreude they really are.

Pretty much everything in the 80's versin of "The Thing". Mainly because the movie was simply an experimnt to see how much people would pay to watch a feature length gore-montage.
I wouldn't exactly call that movie a "gore montage" when there are only three particularly brutal scenes, only two of which involve humans.

There Will Be Blood was really a suspenseful thriller more than a horror. Every time I got comfortable in the theater, that music would just unsettle the shite out of me.

Or the guy that gets frozen over in space...Or the guy that gets melted by only the sun's reflection (or something like it)...

Man, that movie had some tragic deaths...Except the last one..That one was beautiful.

The old Halloween 2 where the chick gets boiled in the hot tub... only movie that I had to stop watching because of a death scene.

- The head in the vice scene from Casino
- The ending to the wickerman (burning yet again)
- And of course the killer rabbit from Monty Python's Holy Grail

That first one was the only part of the movie coming close to disturbing me. The second one, though, was rather hilarious (from narm) in the movie.

On topic, Asuka's death from End of Evangelion. If you've seen it or just browse the internet, you know what I'm talking about.
That was a pretty fucked up death but its something that I expected to happen.
