Seconded. As far as I can remember it's the first time I saw rape in a video game. And it was monster on monster.
Don't forget the first fight with him. Took me a good few minutes to figure out that I couldn't kill him and then he'd just walk off into a soon-to-solidfy staircase. And then later when he and his friend impale themselves. Come to think of it, P-Head wasn't all that gifted. I think his tragically weird martyrdom (on top of the monster raping) was key in defining him as "powerfully unsettling" over "purely evil" (if evil rapes evil is it good?).

I'll accept and raise you Ocarina of time's re-dead. Monster on player rape is worse especially since they paralyze you first. Pyramid head loses points since he supposed to mess with your head.

Never played Silent Hill, although I have seen Pyramid Head in "action" and yes, he is pretty disturbing. Plus, the Poison Zombies and Poison Headcrabs it HL2. There's something seriously disturbing about chunky zombies, and those p. headcrabs, with their hissing noises and creepy appearance. Ugh...

Poison headcrabs always scare the living shit out of me (and lead to massive overkill and ammo wastage) - especially when they jump at you from behind ("...administering morphine...warning, user death imminent..."), and fast zombies can make me go "AAAAAAAAH GODDAMMIT" too sometimes, if they appear out of nowhere (or sometimes if you can hear them coming, e.g. drainpipes in Ravenholm) and run at you, screaming like maniacs, without their skin.

Manhacks also creep me the fuck out from time to time, the idea of some little spinny bladey thing whirring towards you to slice your soft flesh into ribbons..... eeurgh.

I remember in one of the castlevanias, the core is a crucified seizuring skeleton. I think it was Harmony of Dissonance because it had several kinds of those bosses.

black headcrabs everytime I heard that clicking noise I went a little insane

and I know it's not an enemy but in sonic games, when you start to drown that music makes me freaking panic

Leapers in HL2. The first you see of them is when they're right in your face. A few shots and they're gone. And then a few seconds later, they're back.

I'll say the Centaurs from Fallout 3, mainly because even though they have four legs, they don't actually have feet. They have HANDS on their legs!

Freud was an arse (IMO) and probably the most sexually frustrated person in history.
On topic, I'm gonna go with OPs choice, the regenerators freaked me the hell out. There were a lot of pretty creepy enemies in the original Shadow Hearts game as well

Sounds like fun to me.
I think I'm going to boast and say not a single game character has ever tuly disturbed me, my love of horror films and games has left calluses on my soul.
If I had to pick what came closest to freaking me out I would choose some of the more powerful enemies from RE4 (e.g. regenerators, the suits of armour) possibly because when I first played through it I was 13 and both wired AND drunk...
The atmostphere of Silent Hill 2 was creepy, no one can deny that, but none of the specific characters.

Pyramid head. I know, who would have thought of that one? (haha) The regenerators from RE4 were pretty freaky too, along with Alma from FEAR. Elizabeth Greene from Prototype is another good one.
