In STALKER Call of Pripyat...the sound of dogs barking.
"Meh," say those who haven't played a STALKER game, "Why would dogs scare you?" Because in Call of Pripyat, the dogs are pure evil sent straight from hell.

They move extremely quickly and never stop so you can get a clear shot, jump past you when they attack so they end up behind you and you have to keep spinning around, do bleed damage with nearly every hit that slowly leeches away your health, and can take a hell of a lot more rounds of buckshot to the spinal column than a normal dog could, i.e. more than one.

And they travel in packs.

I hate the dogs more than I hate the Bloodsuckers.

I always found that crescendo of wooshing sounds from the Wallmasters in Ocarina of Time really unnerving. I guess it isn't as bad any more, but I still don't really care for it.

And I agree about the turrets in Portal, they are kinda creepy, but I felt that it was so appropriate so I loved it.

If anyone here has played kingdom hearts 2 and has fought sephiroth then they know that the worst sound to hear is sephiroth muttering "Descend Heartless Angel" and seeing a halo pop above your head.

Also the sound of a blue shell in mario kart on the final lap.

This.

That signifies that a lot of work just went down the drain... and yet I come back for more.

Or in terms of annoyance, the Zelda Beep Beep! Beep Beep! Beep Beep! sound when you're low on life. Drives. Me. Insane.
this, or, the sounds of a splicer talking, but I can't see them, and even though i have surround sound, it always seem to be coming from right behind me no matter where i face *shudders*

The constant spout of drivel that comes out of the mouths of people that aren't in the same room as I am when playing a game that supports microphones

When you hear this sound, the dread you feel of what you know is going to happen is only drowned out by the horror of when it does happen. If you don't know what happens and you can stand horror games, go get Silent Hill 2 or 3. Don't even read this, if you like horror, go buy it now.

I hate it when the zombies in HL2 beg for your help. Same as the ones in System Shock 2. It disturbs the fuck out of me.

For me, the witch's theme was always more intimidating than the tanks. Not for what was coming, but just because it was so unsettling.

Another painful song is the Black Hole CO Super Power from Advance Wars 2. It's a very big military level "SCREW YOU" to the player.

For individual sound effects (That haven't been mentioned, most of mine are already up here) I'd propably go for the crying sound from baby Mario in Yoshi's island (SNES version, haven't played the other) for its oh crap and annoyance factors, and the guitar's sounds as you fail a song in Guitar Hero.

That all too familiar sound of a beam rifle firing in Halo 2. When I am playing as the Masterchief and I am surrounded by Marines I quickly realize that the only thing that could have fired that would have been the dreaded Jackal Sniper and that therefore I must find cover and begin what amounts to a deadly game of whack-a-mole as I peek above cover and pray that I can shoot them before they shoot me.

The sound your 360 makes for the first and last time after moving the console with a spinning disc inside.
Rest in peace, Crackdown... :(

First of all:
HOLY SHIT THAT SCREAM!!!!!
OoT ReDeads are nothing compared to those.

Second place goes to the dreaded Click of Death in Counterstrike: Source. Or well, pretty much every shooter. You're in the middle of a gunfight when all of a sudden "chinkchinkchink" OH SHI- And you're dead.
