So what was the most embarrassing way you died, failed a mission, or just plain got a game over from?

For me, it would have to be from MGS2, a game I have just recently beaten. It was after the harrier fight and I had to walk across this pipe. This pipe was covered in seagull poop. I stepped on the seagull poop and stumbled and slipped off the pipe like something out of a cartoon and fell into the ocean. My brother and I both had a good laugh about it.

So what about you guys?

The numerous times i try to jump across the gaps in ctf_doublecross and end up falling to my death, if i have the intel it's even worse!

The multiple times I've fallen to my death because I misjudged a jump in Uncharted 2.

I killed five deathclaws in a row in new vegas, only to die from a radroach pack a couple minutes later.

I was the last player alive on MW2 search and destroy, so everyone was watching. the score was 3-3. And i accidentally shot my own claymore, blowing myself up and losing the game. I could practically hear my teammates' facepalms.

I was kiting some noob in EQ2 pvp, I didn't need to, but thought I would look pro doing it, then preceded to fall of a cliff and die.

I was playing an old game called "XIII" today. I got brought down to 2 health with no health-packs. I got through loads of enemies, carefully choosing cover and taking aim quickly and precisely. I then fell off a 3 foot drop and died. This was an old game so I got put back to the last checkpoint near the start of the level.

When playing GTA San Andreas when doing a mission involving jumping out of a plan and parachuting to safety, I opened the parachute really high up and so had to wait like 5 minutes to get to the ground so I went to get a drink, but as I stood up, I accidentally nudged the Enter key (don't ask how), thus releasing the parachute and sending me to my doom.

In a few games, I've backed up to get a good view of something and walked off a ledge. Far more embarrassing than simply slipping or misjudging a jump.

I think most answers in this thread will involve falling

Most recently it's been in DE-HR. Cleared out an area, taking out guards with ease, then missed a ladder and fell to my death.
I actually facepalmed.

playing left 4 dead on expert mode, i had very little health so i was shooting at any possibly threat
anyway my team mate came up to holding a gas container to put down and shoot at when zombies got near, but when i turned around and saw him right next to me i thought he was a zombie, shot him and ignited the fuel killing us both

I was at a LAN party playing one of the Ghost Recon games with my friends. One of the other guys on my team was going to throw a grenade when he calls out "Frag out!... oh crap! RUN!" For what ever reason, his grenade ended up landing right near us. so I got up and ran... right into the grenade. It exploded right under my feet and everyone there watched as I turned into a crater.

that phrase "Frag out!... oh crap! RUN!" was uttered far too many times at those LAN parties. it took us too long to get used to throwing grenades through door ways without hitting the door frame.

Zork - I'd discovered some of the vaguely amusing commands like "Jump", which said something about you jumping up and down like an idiot. I typed it to show a friend, forgetting I was on the edge of a ravine. "You jump into the hole and fall to your death"

First time trying to do the second campaign on Left 4 Dead. We were running to the boat. I was first on it. One of my team goes down quite far off as the other two make it there. I'm like "NO MAN LEFT BEHIND!" So I run back through the hordes of undead alone, revive this team mate (I think it was Zoey) and we start fighting back through when suddenly, "TAAAAAANK!" We just make it onto the boat, and it starts to leave when the Tank smashes me, sending me flying. I'm somehow left behind, still alive, just downed. The campaign ended, and I didn't get the achievement.

Also, my friend was playing HL2: Lost Coast, and was fighting the helicopter on the scaffold. It's right in front of him, and as I watch he says "come at me, bro!"

It did. It charged right at him and smashed the scaffold, sending him tumbling to his death.
