I briefly played Modern Warfare 2's multiplayer (as in, before the hacking really got underway), and at one point I'd finally gotten enough kills to get a supply drop (I never said I was any good, just said that I played it). So I call it and then I sit around guarding the drop location. Right as I'm wondering why the drop is taking so long, I get killed. Turns out the supply drop landed on my head and killed me. Oh, and someone from the other team got it right afterwards.

In Deus Ex 1 and HR I have died at least 100 times in each from falling. But now that I have the Icarus I just get an annoying animation every 10 seconds -_-. (I don't like taking stairs)

In TF2 I was an Engineer with a frontier with about 10 critz's. I was sneaking up on this solider. I get to a fairly close range where my gun should hit. I fire and they all spread around the target. None of them hit. Like in a fucking cartoon. The target does a slow dazed spin. By then I was point blank about to shoot again. He hits me with his melee weapon. CRITZ I die and slide halfway across the room.

In Halo 2 my friend got stuck on an edge and tried to jump over it. Unfortunately it was dark, he jumped off a cliff. I asked him what happened...and before he answered did the exact same thing (even the same location). This is the same friend I blew up with a scorpian tank on a bridge. He stole a ghost and I mistook him for one of the many enemies. He also failed to board the scarab and respawned over the edge and fell off again. Good times. Good, hilarious times.

World 1-1,
Frickin' Goomba.

Thankfully that's only happened once, it was still disgraceful.

Some guy in runescape told me that he knew where the runesword was. I was level 2 and I followed him into the woods where he slaughtered me like a pig.

I quit the game and never played again after that.

When I first started up Deus Ex: Human Revolution the first thing I did was run up the stairs to the next floor. Realize there might be something done on the bottom floor.

Then think to myself I don't want to go back around to the stairs again, so i'll just jump off this railing here, it's only what 2m.

Jump and inevitably fall to my death, oh what a sigh I gave, thinking I guess Jensen needed more time to recover.

Playing Mass Effect 2 on Insanity mode, I thought a lone Blue Suns trooper would be no problem. So I went out of cover, stormed at him while he sprayed me with his assault rifle, brought my shield down and killed me with his incineration blast.

The first time I tried to play Oblivion, I didn't set up an auto save function. Played for about an hour, was finally getting used to it. Out of nowhere some wizard popped up and started attacking me and I just barely managed to kill him. Then, while exploring, I decided I was superman and leapt from a rooftop to my death.

Oh and one time I was playing MGS, on that floor with the trapdoors. Was walking around, paused to think about something, fell to my death.

Then one which I think is more embarassing for the game. I was wandering around in GTA:VC and was standing ankle deep in water trying to decide what to do next. I looked away from the screen while thinking, and when I looked back I had dropped dead. Apperantly the water around vice city has a PH of either 1 or 14.

I've also had times in the various God of War games where I'd have to jump and grab a rope and Kratos would just hover next to it, staring at it before falling to his death.

And lastly, in Dragon Age: Origins. I once decided that if I ran really fast I could make it past those fire barriers in the fade in the mage tower quests. I learned the hard way that, no, you cannot do that.

So what was the most embarrassing way you died, failed a mission, or just plain got a game over from?

For me, it would have to be from MGS2, a game I have just recently beaten. It was after the harrier fight and I had to walk across this pipe. This pipe was covered in seagull poop. I stepped on the seagull poop and stumbled and slipped off the pipe like something out of a cartoon and fell into the ocean. My brother and I both had a good laugh about it.

So what about you guys?
Having half a heart of health in GTA3, trying to steal a grannies car and ending up dead because she fought back with her walking stick. I just laughed, deficiently the most anti-climatic and pathetic death I can remember having.

Operation flash point 1985,

AI: "OH no 4... is down, Damn 3.. is down.."
Me: WTH? oh jesus! SPLAT!

Me and half my squad flattened at the bottom of the hill - by the Blackhawk that we parked on top of the hill, but clearly forgot to put the hand brake on.
LOL, once I was flying a CH47 (In ArmA 2: OA) in MP and was going to pick a mate up, but as I landed he ran UNDER the chopper, and well..... lets just say he wasn't getting back up anytime soon.

Another funny death in the ArmA series (this time ArmA 1), my mate and I was playing Capture the Island, so we had cleared out the town and was waiting for the capture to finish (one of the major towns, I remember we use to use the name paris as all the towns had our own names due to them being hard to pronounce half the time, this one being called paris since it was large and started with P).

Anyway, so we had checked the town like 5 times and there was no-one around, so my mate was on a platform/2nd level next to the town capture structure and decided to run from the platform into the structure (no jumping in ArmA), as they were close enough to do it. Well he starts the run and yells "I'm gonna make it", a microsecond later we hear ONE round fire, hits him and kills him outright. His body falling so that his legs were on the platform and his arms/head was on the structure. We pissed ourselves laughing for like 5 mins, then my brain jerried I was standing up and exposed so I took cover. The best part is after he respawned and came back we searched the town and found no-one around, never knew who fired the round. He kinda made it lol

Another time, in Rainbow 6 Vegas 1 the same guy as the one above and I were playing coop campaign. We were on the vegas strip, just going into the building with the double metal doors. We had played this mission to the point of knowing it back to front and so knew there was a guy on the other side facing the other way. So I said "Open the door, I'll throw a granade", he does the cover, opens the door, then I throw the granade.... as he leaned around the corner to look. The grenade hit him and just dropped to the ground. I looked at it for like a second or 2 in shock/horror and then my brain clicked and I turned to run. I got about 2 steps when it went off and threw me head first into the wall down the hallway. We laughed our heads off, my mate saying "You know your f##ked when you can clearly read M68 hand grenade on the side as it hits you and drops to the ground next to you" He didn't even bother to try and run.

It'd probably being playing MAG on the Valor Sabotage map. Trying to take an objective with the whole enemy team defending it. We had the front door with a supply crate. It's at the bottom of some stairs and there's a small hallway with another small staircase leading to the objective. So, I threw a grenade hoping to clear out or at least damage a few enemies. I then sprinted down the hall and died. In my own grenade. Kinda forgot I threw it in the heat of the moment. I've actually done that a few times. And you always feel stupid when you do.

Death on Remelia Scarlet on Touhou 6. Normal difficulty, Final Stage, Down to her very last spell card on her last life bar. SO FREAKING DAMN CLOSE!!!! I don't even know when I'll ever be able to replicate getting that far. But I've got one life in this universe and beating this game is on my bucket list.

Another good one was when I was playing Super Smash Bros Melee with a friend. We were on the Rainbow Cruise stage and I was Kirby. At the top of the stage I grab my friend and I toss him into the air with Kirby's signature meteoric smash. Only instead of returning to the ground, Kirby registered so high on the game's engine that I actually flew off the screen in attempt to toss.

I was at a LAN party playing one of the Ghost Recon games with my friends. One of the other guys on my team was going to throw a grenade when he calls out "Frag out!... oh crap! RUN!" For what ever reason, his grenade ended up landing right near us. so I got up and ran... right into the grenade. It exploded right under my feet and everyone there watched as I turned into a crater.

that phrase "Frag out!... oh crap! RUN!" was uttered far too many times at those LAN parties. it took us too long to get used to throwing grenades through door ways without hitting the door frame.
I know that feeling, many a times we've had a grenade land next to us in ArmA, Vegas, Ghost Recon and other games. I swear you never see someone run faster than from a friendly grenade

First time trying to do the second campaign on Left 4 Dead. We were running to the boat. I was first on it. One of my team goes down quite far off as the other two make it there. I'm like "NO MAN LEFT BEHIND!" So I run back through the hordes of undead alone, revive this team mate (I think it was Zoey) and we start fighting back through when suddenly, "TAAAAAANK!" We just make it onto the boat, and it starts to leave when the Tank smashes me, sending me flying. I'm somehow left behind, still alive, just downed. The campaign ended, and I didn't get the achievement.
*SNIP*.
That reminds me a time when playing in L4D2. When you first call the boat at the mansion. We were doing the final run to the boat, I got to it and so did one of my mates, the other one was coming but a tank caught him, We've gone "HURRY UP", so the tank whacks him and sends him flying, and somehow landing on the deck ending the mission successfully.
