I know for SURE that it's based upon a book though. I remember the line "Hold my breath as I wish for death". He does this in the book, but they attach a tube into his throat so he can't. Then (I forget how) his nurse manages to communicate with him. He then lets her know that he wants to die, and she kills him.

I'm pretty sure that the song is based on "Johnny's Got His Gun", so yeah, you were right.
