The hand that falls in LoZ:Link to the Past the dark wood temple. It just wastes time by bringing you to the start.Also I hate its return in LoZ:OoT in the Shadow temple I thinks it wall master or something. Its just annoying 
regenerator from RE4 and Deadspace. Since Deadpace, while an awesome game, was a comeplete rip off of RE4, the regenerators in both games are most likely the one and the same. They both make wierd noises and can still attack even when they're missing several limbs and most of all, you spend half of your ammo killing them.
That water temple boss from OoT. I remember playing that when i was like, 10 and almost shitting my pants when i would fall into the water which was alive and trying to kill me.

I'd have to say the chainsaw guy at the beginning of Resident Evil 4 on professional. No matter how many times i blast him with the shotgun and grenades, he always manages to hack Leon's head off. I swear i watched it like 8 times before i gave up on beating professional.
