I recently won Osu! Tatakae! Ouendan 2 in an ebay war, and because I know about as much about the Japanese language as Dan Brown does about Christianity (Heyoooo!) I found the first ALL OF IT impossible to get through.
Then I completed all but one of the difficulty modes and unlocked a particularly sadistic one where you get no timing circles.
In a game based SOLELY ON TIMING
(pause to let that sink in)

Choosing the one most frustrating moment is a really hard question, because while games are fun, they can be horribly frustrating. The first time I broke something while playing a game (I've only done this twice) was during Gears of War. I turn this corner and see a guy, aim, shoot, aim again, shoot (with shotgun very close range) he turns around and a grenade goes off causing both of us to flinch. When flinching is over he begins to bring up his bayonet, and I get one last shot off, direct hit that's even closer than before and he chainsawed me, didn't even flinch from the shot. I threw my headset onto the ground and it broke. Other than that CoD4 and World at War are both incredibly frustrating yet addicting.

Ok, maybe it was just me back in the day, but the two headed dragon in the mountain cave in FF7 was the most frustrating, annoying fight of unholy controler throwing I've ever experienced.

Oblivion. Everything is too big and generic, the main quest is stupid, and the game wasn't good enough when playing with a thief character.

EDIT: Oblivion might not have been the most frustrating, but I didn't like the flaws.

Dying in PoP: Sands of Time is very frustrating too

Suddenly seeing that the genius that was Shadow Of Cherynobal was turned in to complete kipper shite in Clear Sky. I am sorry but having to take a base from an enemy that out number me 14 to one, all of whom are heavily armed, have super armour, have super accuracy, can take 4 four point blank shot gun rounds to the head and still keep going and don't have to worry about the stupidly implemented take one shot and then bleed to death issue the player does... well annoying is the word that comes to mind.

Constantly losing in PvP battles on WoW and then being tea bagged and screeched at with words you can't even comprehend because they're so horribly spelled.

Ninja Gaiden II. Where, specifically? Everwhere. No other game has consistently made me rip out my own hair. The odd thing is that I love the game.

Trying to aim in Bioshock was hell for me.
This, I have no idea why, I just found it so hard to get any head shots.

The first time I ever got frustrated at a game was unlocking the last character in Tekken 1, so I took out the disc and bit it.... (I was no older than 10).

Playing through No Mercy on expert just to die three feet from the helicopter then have the rest of my team run right by me into the helicopter. No achievement for me....

Only when you ran out of sand, otherwise it was only a minor annoyance.

My most frustrating moment was a long time ago. I owned a lot of crud games and the horrible interfaces always made me angry. Can't remember a specific time though.

Old Olney in Fallout 3 on Very Hard. I was so tempted to use the Fatman for most of the Deathclaws in there. That's a recent one I can think of.

The winner for all of time and space however was Jumping Jack Flash on Elite Beat Agents. Cruisin' difficulty. DO NOT DO IT IF YOU ENJOY HAVING A LIFE.

Oh, and

People who park their cars on the track on racing games just to block the people who are trying to race also the ones who take other people out on purpose just to get the position.

Also just trying to get past the hardest songs on rock band drums. Run to the Hills was so frustrating but so rewarding after finaly completing on expert.

Oblivion. I only bought it 2 weeks ago, and luckily I hadn't done anything except Thieve's Guild missions, but when I got to the final one where you need to steal the Elder Scroll for the Gray Fox, I was caught by the blind monks and had to escape the palace without the scroll. I now can't get back in and the game won't let me finish the mission. Fortunately, because I haven't done anything else yet I can restart the game and not actually lose anything major :D.

Ino the first time you play Guilty Gear X2 through arcade mode. I was up for like 2 hours with a blood vessel ready to burst before I beat her, got to the point where it wasn't fun but I NEEDED to see the ***** on the floor before I could sleep.

When GFWL went down last year I played through about half of gears on insane difficulty, when live went up it loaded my previous save near where carmine gets shot. I was on the train before :(
I was pissed.

recently i have been playin g gears of war 2 and i'm on the part of the campaign (on insane difficulty) where you have to go threw a tunnel filled with tickers and dizzy is behind you barely lighting the way and tickers pop out of you ass and your instantly dead.
Shotguns are your friends. Plus, if you're fast enough you can bash them away before they explode.

It's an achievement to bash Tickers 30 times, I believe. Not the same one repeatedly, obviously.

Oh, and the final part of Rainbow 6 Vegas 2 on Realistic. Finally beat it, only for it to NOT GIVE ME THE ACHEIVEMENT. ARGH!
