The crabs and mantises from Metropolis Zone on Sonic 2 are far too well placed.
The pirahnnas in Hydrocity in Sonic 3.

Let's see... From the top of my head;
- Wallmasters from the Legend of Zelda OoT and MM. They were rare, but GOD, I hated them.
- Creepers from Minecraft, they always ambush you, when you're building something beatiful..
- Those enemies from the Metroid Prime triology, which flies right at ya' and explodes. They don't do a lot of damage, but are almost unavoidable.

Any enemy in Fallout 3 with an ungodly health bar. I don't feel like emptying 10 mini nukes into one target just to cripple its legs.

Recently, I've begun to hate Crushers and Titans in inFamous 2. The game never tells you how to kill Titans, not even a damn tip. If you don't know what you're doing, you'll spend a lot of time trying to kill those things. Crushers are just generally annoying and difficult to hit.

You two do realize that if you put a bullet or two in their leg they couldn't dodge for like 5 seconds, right? o_O The game even tells you this once you start meeting the dodging ones...

Anyhow, my vote goes to those Goddamned Bats [http://tvtropes.org/pmwiki/pmwiki.php/Main/GoddamnedBats]. ANY enemy that fit into this trope is most likely gonna annoy the hell outta me.
I did realize that after a few attempts at leashing them, but it just made it more of a hassle than I thought it needed to be. Although, now that I think about it, I guess they needed to add some sort of challenge so that you couldn't just leash-kick every enemy you saw.

JESUS H. CHRIST, those guys are relentless. Damn near impossible to take in a group of more than 3 and to make matters worse, Vergil annihilated a dozen of these so freaking easily.

Really? I never had much trouble with them on any difficulty. They do have that annoying leaping attack like the red guys in the beginning (lust?) but they make a sound before they do. Just never stay in the same spot for too long and you will see them miss their leap all the time. Ditto for the ranged attack.
Fun fact: if you hit them with cerberus icechunks start appearing through them, no idea why though.

Now those angel-ish guys (fallen?), those were pretty annoying. Simply because they dragged the fight on forever by rarely opening their shield and hiding in walls. Near the end there are a few who you don't have to kill to open a door so I just walk by them. Block that, jerks.
Also: greed, the guys who summon more guys. The answer is simple: focus on them first. But on dante must die they have so much health that by the time you killed them they summoned a freaking legion.
Hmmm point well made. The Fallen were indeed damn near impossible. Kinda forgot about them.

But if I remember correctly I think the Abyss were weak against Cerberus

OH damn man. I was scared like a little ***** when I ran into them and didn't know how to beat em.
And to make matters worse they have that goddamn heavy seizure like breathing and the music makes it worse.

Those enemies gave me nightmares
