I have watched someone do it, it took him a while and a lot of deaths.
Its not the needler you need to worry about, its the "Snipe from so far away you cant see me while in the middle of a jump in a vehicle" guys you need to worry about. On legendary they aim for the head and hit 29/30 times

The first game I owned for my PS2 was Haven: Call of the King (or something like that). I got as far as a part where you have to fight against a massive pirate ship using a mounted cannon on a small boat. I never beat that part.

the end boss fight in R-Type 3 I thought was pretty unforgiving. You get machine gunned by giant energy balls which go off in random directions, so is different every time. Also the last checkpoint is miles away.

DMC4 of dante must die, Couple the amount of damage you take, with how much health the enemies have and the surpising amount of times you have to fight bosses (the same boss over and over and over...) which is fun on the easier difficulties but a complete ***** on DMD, oh and pretty much every enemy can regenerate health.

Mortal Kombat 2 on PS3
Actually, it's hard because of insanely cheating CPU. Ninja Gaiden and God of War on max difficulty is easy compared to MK2 on medium. And that's insane.

I'd like to say Killing Floor on 6-player co-op. Done a couple of matches, the Patriarch is ridiculously tough.

Earth 2150 was a difficult game. The campaign is simply merciless, it takes some real thought to get anywhere

Ninja Gaiden:Black was the first game in my life I ever had to put down and admit defeat. But I'd say for me personally the hardest game was Sniper Elite. I wanted so badly to do good. But I couldn't beat the first level. I'm simply too impatient for it...

S.T.A.L.K.E.R. SoC on "master" with some reality mods to make it even more realistic i.e. even harder. You'll never believe how many times I had to qiucksave/load in this game.

On this one, I have a few examples of particularly tough games.

Persona 1 (US) was undecided. It started extremely easy and the difficulty ramped up steeply beyond the second half. Last boss was at the end of a one-two hour dungeon (since the last save point) and can pretty much one shot you. Maybe I was underlevelled.

Wizardry 4, Return of Werdna was extremely (and unfairly) difficult. Some of this was due to its old school system, but not all. Enemy rogues can steal important quest items from you, returning them to their original spawn points. Some puzzles required prior knowledge of the Wizardry series to answer. One floor is a maze that changes as you move through it, and can change in such a way to box you in and can't change it back. Combat was also consistently challenging throughout. Save and reload tricks rarely work either, as this causes all enemies to respawn. It's been dubbed the hardest rpg out there and I agree.

I remember trying Dragon's Lair for the NES as a kid, didn't go well...
Other difficult games on the NES I remember are Ghost 'n goblins and Battletoads.
