Gothic 1:
The developers decided to be lazy and programmed a scripty thingy to kill your character if they are falling and are above a certain height.
This sounds nice in theory but I found was that your character could fall for 200 meters without touching the ground, provided you could find a slope with the correct angle. It was also revealed that falling more than three feet will kill you, no exceptions.
I also found that sliding down a slope is half and half of those two, so that if you slide down a slope and drop onto a patch of flat ground that is under it you will survive, but if you have to jump a gap to land on the grass... well think of Portal when you set up that eternal fall thingy, then you turn off fall damage, then shoot a portal so it faces a wall......

Deflected a rocket that curved away from me at 270 degrees, into the wall I was standing next to for cover. The four witnesses proceeded to bury the chat channel in WTFs.

Can't remember which game, but I slipped through a edge between the floor and the wall and went falling through the void without dying.
Pretty sure it was an old game.

like.. Blue Dragon, saving inside the Nene's Fortress at the end of Disc 2 and not being strong enough to beat the bosses..

There are a bunch of those awesome vids. For example:
This guy tried to snipe another guy on Last Resort. The bullet bounced off the pillars and he got a head shot... on himself.
Another guy stuck a Brute on the first mission. It then exploded, propelling the now corpse to smash into the player. According to the game, he was run over...

As for me, I'd have to say any escort mission. Damn stupid AI...

while this was not actually a death, I can remember once while playing the legend of zelda: majoras mask.

I was in the ice tower/temple/something and I got hit by this wherewolf thing at the exackt same second as I opened a door.
What happened then was that the door now pushed me trough the floor and made me fall into a vast void of pale blue nothing until the game decided that I had fallen into a chasm and then respawned me... ...under the door.

Like this it went on with poor little me not having any means to stop it, other than turning the game of, then on again and continiue from my last save, with just happened to be right after I had completed the swamp temple a couple of hours back. '

fun times. Fun times indeed.

Like Yahtzee I gotta go with the lego games, In lego star wars on the PSP some stupid npc stood on the end of a plaform, constantly knocking me off and respawning me so I fell down again, I had to re - do the whole level :(

Tomb Raider 2. you do this little code

"While playing hold R2 and press Left, Right, Left, Up, Down, release R2 and hold Left to spin five times, then press Down + Square to do a backward jump"

Then you explode and it's game over.

Fallout 2 : I was in a town and I accidentally got into a fight with one of the NPCs, then a kid threw a grenade at me for about 100 points of damage.

Every time you get killed repeatedly with melee weapons on Team Fortress 2 because of the ridiculous crit ratio.
Seriously, how can a bat do more damage than a flamethrower!?

in halo my own marines are always out to get me by a face full of lead or suicidal driving. so i may shoot them in the head... and they put up more of a challenge than the covenant.

In MGS3:Snake Eater I decided it might be a good idea to randomly try one of the mushrooms i'd foraged...while half way up a facking huuuuge ladder...mushroom put me to sleep...fell off ladder...hadn't saved in hours...turned game off...never turned back on.

P.s. Just remembered, just captured my first "card" in Mercenaries. I signalled for the chopper and threw the prisoner in when it landed. Cue the slow motion and heroic music...and then the chopper knocks me off the cliff i'm standing next to with it's tail rotor.

It's like they incorporated "heart attacks" into the game or something, it happens very rarely - probably once every 100 games, hell, it happens at "convenient" times, and by that, I mean it doesn't happen like...while you are carrying the enemy flag or something.
I remember on XIII's multiplayer on the PS2, you could die from heart attack, and the game would actually recognise it, saying "so-and-so died from a heart attack"

Personally, I'd have to say my stupidest death was when I was mucking around on Gmod, and died walking past a dead body. I can only assume that Freeman needs new specs to avoid tripping and breaking his neck.
