You're actually just in a video game! Trippy!
That was precisely the point I put my controller down and never played the game again. As trivial a video game is, I felt cheated.

The best plot twist ever in any format though, would have to go to Ender's Game. I must've read that page 3 times in a row before I finally realized I didn't read it wrong.
Hell yeah, that really got me too in Ender's Game. One of my favorite books btw.

As far as games, I gotta go with Xenogears and the whole personality subplot of Fei's. Especially at the end of Disc 1, epic. Playing it a 2nd time I couldn't believe I didn't see it sooner, but the 1st time though I guess I just wasn't paying enough attention. In fact there are TONS of twists in that story, probably why it's easily my favorite RPG of all time.

Movies...gotta go with The Usual Suspects. The definition of mind blowing plot twist.

The plot twists in the Ace Attorney series have surprised me many times each case usually has an interesting plot twist

Mine was also from Bioshock. The moment when a certain character was revealed to actually be a certain other character. I was completely blindsided by that. And I usually see these sorts of things coming from a mile away.

Shit, I forgot quite a few good ones
(inFamous)
Hell, just look at my damn username. I can't believe I forgot that one.

The end of Shutter Island was fantastic. (The book, I haven't seen the movie.) It took a few seconds for me to accept that I was out of pages. I thought there had to be more. Christ, what a mindfuck.

aaaaaand....(Memento)

Speaking of mindfucks, I have one word for you: MEMENTO. Adding a few more: HOLY FUCKING CHRIST, THAT SHIT IS RIDICULOUS! (I mean that in the very best way possible) "Now, where was I?" Fucking awesome.

and, hey, as long as we're not confined to videogames:(LOST)

LOST. I'm just gonna list all the best ones off (from ALL 6 SEASONS): There's a dude living in the hatch, there's a giant boat in the middle of the jungle, the others live in a nice little neighborhood where they host regular book club meetings, it's not Penny's boat, it's not Locke, either, speaking of which: Locke was in a fuckin wheelchair, they time traveled when Ben moved the island, the monster is an ancient pillar of black smoke/shape-shifting-human-hating-entity, and Jacob is his mysterious anti-thesis. (Takes huge breath) The island is underwater, the bomb somehow worked and didn't work at the same time, Jack has a son, Claire is his sister, Claire is insane, there's a mysterious lighthouse that can spy on anyone in the world, WIdmore used to be an other, there's a damn hydrogen bomb on the island, IT'S ACTUALLY A FLASHFORWARD, THE OTHER THING IS ACTUALLY A FLASHSIDEWAYS, and, of course, the origin of Jack's tatoos is that nobody cares. Thank you, thank you.
Damn you for ninjaing Memento, that movie was great in an insane way. It just drew you in so well.

Has to be the spice girls...

So sad when Geri left... :(

But really now, i liked the one in mgs2, where you find out everything is just a simulation, and things start flipping out around you, and the npc's start talking to the player instead of the the player character.

For movies- Memento. Won't ruin it for anyone

for a video game, I would have to say KotOR. It makes sense when I go and play through it all again while knowing it, but I was shocked at least. And I won't spoil it for anyone that, for some reason, hasn't played it yet. Even if its been ages.

Also I should also mention
The Sirens of Titan- just because its one of those plots, in a novel though, where none of it makes much sense at all until the end- and then it all suddenly makes sense.

Dante's Inferno
The fact that it's actually the 1st of a line of games and will have a sequel! Gasp! Maybe it'll be a trilogy? Hmmm, a hack and slash where you play as a guy who brutally kills everyone he meets on a quest to redeem himself to a God, where else have I seen a trilogy like that?

There are others out there, just not fresh in my mind like this one...

Erm, spoilers? I mean, I've played it, but some haven't.

OP: I like the KOTOR one. I won't say any more with fear of ruining it.
Thats the only one I can think off xD

There have definitely been some games and movies with great twists.

Hotel Dusk is probably one of my favorites when...
You find out that Dunning, the owner of the hotel was the man who made the famous 'Osterzone' paintings and that he was conned into doing more art by his former best friend, Mila's father. Bradley's message to Kyle about his reasons for betraying their precinct, and finally Kyle bidding the hotel farewell, with Mila tagging along. The bonus ending where Dunning's daughter, Jenny reappears took me by surprise as well.

Freedom Force 1 was pretty sad since
Man-bot was forced to remain in the great clock since he absorbed it's power and couldn't leave, and that all he had to do for eternity was think about how his powers lead to him kiling his brother.

Like a lot of people I did not see Heavy Rain's twist of
Scott being the Origami Killer. But I have to say it does make a lot of sense in more ways than one after taking the time to think about everything in hindsight.

Matthew Reilly. Author. I recommend him to everyone.
Anyway.
Scarecrow
In Scarecrow, when he kills off the main characters lover, after two books before this, and they were going to get fucking married. And he beheads her. With a guillotine. It was horrible.
Jack West
Also, in the Jack West books. The third one again. Wizard, West's mentor, once more in this from the start... Actually much before. Gets shot with a crossbow. Again, extremely sad. And fucking unexpected...
