Which character's death had a great story impact or freaked you out? At the top of my head, I have two death. Modern Warfare when Gaz got shot and MW2 when Roach and Ghost died.
Yeah they got me too. Something I wasnt expecting a plot twist like that from MW2.

However I have to admit a few deaths have had an impact on me. Seeing your dad die on Fallout 3 also had me go "NOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOOO"
yes this and:
It was my fault, as in it wasn't scheduled into the game per say, but it was allowed. In the National Archives(F3) I accidentally killed Sydney with a flak grenade. Not my fault at all...

Galuf - Final Fantasy V

He stood up to Exdeath with the last of his strength, much more moving than Aeris (FFVII)

First time I played FF VII my friend told me to level Aeris and use her all the time. I liked that she could heal everyone with her limit break. But she died and I asked my friend if I could get her back. He laughed and said, "she is dead forever! Muah ha ha ha!"

My friend was a dick.

Duncan in Dragon Age Origins
That guy rocked!
And you can't even take his friggin awesome armor!! =(
I like to call him Jesus (mostly because I couldn't remember his name). He also sort of looks and plays the part.
Jesus didn't tackle ogres and stab them to death. If he did, I'd be Christian.
Yeah, I know. I just thought it was rather funny how bioware added two Jesus like figures into their game.

The Darkness

Your character Jackie is forced by The Darkness to watch your vengeful Mob boss and a corrupt police chief shoot your girlfriend Jenny in the back of the head right in front of you as she sobs and whimpers that everything was going to be okay. Then you commit suicide. That was probably the biggest punch to the gut I've ever seen in a game.

Well...

Borderlands

T.K Boha(not the worst, but that's when I knew the game was going to go downhill after killing a important character, without any particular reason.)

Gears of war

Kim, simply because it was dramatic, and well done, and B Carmine, because he was a good character

Can't think of anymore, aside from Fallout 3, COD and Half Life 2. But they've already been mentioned
TK isn't dead. Sorta.

I've gotta agree with this. When Roach and Ghost died I hated Shepard...what the hell was his motive? Why would you betray the Task 141 when a fucking crazy Russian terroist is on the loose?

Oh, and I suppose the death of Bionic Commando's spouse was surprising, if not a little strange. I honestly didn't see it coming.

Saints Row 1

When the saints had to kill your playable character when he was on the boat with that guy. It was pretty epic to see them.

Even though you come back in SR2, it was still epic as hell

BOTH, in inFAMOUS
Trish was very sad for me, even playing as good, knowing it was coming, and KNOWING i couldn't save her without her hating me for it.. saddest
AND then, motivated by the above, i end up killing kessler, only to learn that it was cole, that was a real surprise.

Agro doesn't die. But it does punch you in the gut for a second there, doesn't it?

All of you who said you were surprised by MW2, you are ninnies. Everything in MW2 is cliched and telegraphed.

I was going to say Luis' death in RE4 (honestly, i didn't think they'd kill him off before he'd done anything noteworthy) but in conjunction with the above statement it would seem like I was just trolling the board.

So... hmm... I can't think of any that surprised me besides that one, so I'll go with saddest.
I give that one to The Bitter Shrine. For those of you less obsessed with details than I am, The Bitter Shrine was the location hidden behind the Bandit Camp demon door in the original Fable. The demon door, after ages of sitting, wants only to see his three friends again. When you open the door, you find that the three friends he's been wanting to see for years have been there all along; they all killed eachother trying to seize the demon door's prize for themselves. I can't even properly explain why, but that is just the saddest damn thing I've ever heard of.
