Just to be different, I'm going to say The Dahaka from Prince of Persia: Warrior Within.

This badass dude will come crashing through a wall at any time and start hunting you down. The closer he gets the more the color gets washed out. If he gets close enough he will fucking absorb you. To get away you need to do death defying jumps across pits and running on walls etc.

I know I'd be scared shitless if I saw this guy

When I was a kid, one of the first games I got for PS1 was Syphon Filter, and I remember how my heart was pounding when I fought a boss named Anton Girdeaux.

He slowly walks around inside the Freedom memorial of DC with a flamethrower that could kill you within a second, and his only weak spot is his fuel tank - but I died a couple of times before figuring that out, my heart beating a little faster every time I died.

There was just something extremely terrifying about his slow movement and the "danger-meter" that would suddenly freak out when you got in his line of sight.

Or maybe it was just the fact that he constantly mocked you with a french accent.

There is only one enemy I consistently run away from in the Final Fantasy series. I'll bet most of you can guess which recurring enemy it is in two guesses. Here's a picture:

He may be so damn cute but depending on the game, a stab by its knife will deal 90% damage, 9999 damage, or automatically kill you. It also has some pretty decent HP and defense in most FF games, meaning it's difficult to kill.

Terrifying looking? Not in the least. But I do run almost every time rather than fight so that does say something about the fear this thing puts into me.

And I am not alone here. Here's a picture from DeviantArt that about sums it up:

Tonberries never gave me much trouble, but I HATE Marlboros (Sp?). You don't even get a chance to fight those jerks since they render your entire team useless with one move.

youtube (equals sign) (insert that last bit of random numbers and letters at the end of the Youtube URL here) without the spaces and with brackets :3

Should end up looking like this:

OT: The goddamn Regenerators from RE4!

Ribbons and/or status effect nulling armor reduces the once-fearful Marlboro into a mere pest. I never go anywhere in a Final Fantasy game without at least one party member immune to status effects and one immune to colour magic.

You reminded me just now of the Fast zombies in Resident Evil REmake. o_o Bastards always made my skin crawl right out the back door.
You thinking of them crimson-heads in the GameCube remake? Cuz if so... Then I totally agree.

Whatheshit?! it got up... okay equip pisto... ITS RUNNING o.o oh shit oh shit... CLAWS D: COME ON... nooooo *failed attempt at survival horror*'
Some time later again... 'oh god I hope that thing doesnt get up... please god don't let it get up x.x'

The gorillas and the mummies from the original Tomb Raider. I DARE you not to shit yourself whenever you encounter those fucks!
I had almost forgotten about the mummies, brr. I think that even now I'd piss my pants in that one place where you can HEAR the mummy breathing in a corridor ahead and you KNOW it's going to attack the moment you set foot in there.

What else... Relatively few enemies invoke this feeling in me. For now, I can't think of more than the colmillos in Resident Evil 4, which I guess it's kind of strange since there's stuff like regenerators and iron maidens running around. But, the colmillos are so fast, so I panic, then I frantically shoot them, but I can't hit them and then WHOOSP, doggie's already eating my neck.

One particularly traumatic encounter with a Rumbler from System Shock 2 used to cause me to shit bricks upon seeing a glimpse of them, but I overgrew that fear.

Mercs in Crysis and FarCry. When I realized that they'd seen me, I'd run like hell back across half the map until I lost them, at which point I would just sit still for about five minutes until I twas sure that they were gone. It's not that they had tentacles or were like the guys in Amnesia or anything, but I was totally helpless before them, and that's not a good feeling.

Bloodsucker from the Stalker series.

You hear this weird panting and footsteps, you see a shimmer and start spraying the place with bullets. Thinking you've killed whatever was causing the noise a smug grin appears across your face as you reload, and then you get ripped apart by Zoidberg's evil, invisible, brother.

Sure the words 'You are dead' immediately appear across the screen and you just reload your quicksave, but thats not the point.
Seconded. It's even worse in Clear Sky where they can't be damaged unless they're visible. :|

I say it's the damn Controllers. You start hearing this ear-splitting beep while ghostly apparitions of your past fly at you. And when you finally do find out where he is, he's been aware of your presence since you entered the building. Good luck trying to hit him while he tries to rip your mind out of your body.

Damn I hate those things...

Honorable mention noted. :) Though, because I couldn't see half the time, the whole game was something I loathed to approach. ;)

The Crimson Heads... I always gave the corpses a wide berth. Something about a ZOMBIE corpse that didn't disapear gave me pause. I had also burnt a few when I noticed I could get kerosene from bins. But when they never reacted, I gave up on them. ALWAYS gave the corpses I encounted room though. My first encounter with a reanimated zombie though, I noticed it looked... more dead. I thought, "hey, he's rotting. COOL!" Then I went on. But then the thing stood up, and I screamed! O_O

Bosses from Resident Evil games kept me plugged in , but mostly because I was also just a kid at the time ( talking about RE 2 & 3 ) . Added to the fact that they were packing an almost one-hit-kill and ammo and health were very scarce in those games.

Anything with unlimited health, e.g. the Hunter from Dead Space, just because the damn doors take so long to open and there's a fucking monstrosity healing itself before running at you while you're humping the door trying to get it to open faster!
