Well there the first time I encountered the super mutant behemoth at galaxy news radio; "hmm that was a lot of mutants I just murdered, but hey look at all these guards at least I'm sfe here, wait why did that bus explode...oh shit what is that thing? Oh cock why is my hunting rifle not effective?" Anyway I found the fatman on the dead paladin and that was enough to subside my fears about fighting this creature...I don't really get that thrill with any of the enemies anymore though granted when I happen across a swarm of super mutant overlords armed with tri beam laser rifles I cry a little...try getting through chevy chase on you're first go with an assault rifle at level 30...I think I used about 50 stimpacks.

Another OH SHIT! enemy was the Leviathon from Resistance 2...now it's not as scary as it was when I first did it.

ALL the enemies from Abe were terrifying but no more so than the Scrabs...the screeching and the victory dance they did when they killed you. So yeah 50ft Monsters are not teffifying at all but a 2d torso with fingers for legs and a mouth that's what fear is.

TOTALLY AGREE. When I played this part for the first time I was about 12 or something, and it was the middle of the night and Iwasn't in my own house. I didn't sleep.

My votes going to the Chameleons from Resistance 2. You have very little warning theyre around until they attempt to one-hit you from about 10m away. If you happen to be facing the wrong way, youre expired.

Thats an Iron Maiden. Regenerators are the ones without the many come-out-of-nowhere-to-insta-kill-you-spikes.
Whoops. I looked up 'RE4 Regenerators' and that's what appeared. Man, I've gotta play that game again...

I found the Ichthyosaurs in Half-Life to be truly scary. I mean, they're pretty creepy to look at, but the main reason is that they're so dangerous. They move fast. They make that... sound (which I don't even know how to describe) when they're barrelling towards you to try to bite you in half. And you have to leave your comfort zone in order to fight them, because it's very difficult to kill the damn things from above, you have to get in there, in the murky water, where it has the advantage. Also, most of your weapons don't work underwater.

And then there was that part where you had to navigate the slippery, crumbling walkways above the pool occupied by two or three of them.

"Don't slip, don't slip"
*slip*
OH GOD

Seconded. I mean, just look at it. And the only efficient way to kill them is with a bloody infa-red scope!

Also, Deathclaws.

The Grinning Colossus. Your weapons don't hurt him! Plus he's there, grinning. And being big.
Scared the crap out of me too. Took awhile to figure out how to defeat him.

I tried to decide on my all time 'Goddammit I hate you, you b*stard' monster, but could only narrow it down to my top three:

EDIT -

that spider from limbo. not only did it make me think i had actually died (when it wrapped me up in its webbing), but when i finally kill it, it makes me think its still alive and has me too scared to go near it! not to mention going near it, ripping its last leg off, and then rolling its body in a ditch so i can continue onwards
That low frequency droning sound that accompanied it was unnerving as well. Visually, I thought the Kraken from GoW2 was mindf*ckingly terrifyng. it would scream and flail, and well on my first encounter cost me a few lives.

After that, calamari.

The monsters in Amnesia by a long shot. Especially how shockingly fast they are once they see you, and that sound that starts playing when they've noticed you. Nothing scarier than wondering around and just hearing that sound, when you didn't even know there was a monster nearby.

Redeads from Ocarina of Time denied me the pleasure of playing it past the third temple. That noise that they make is just...Ugh.. I finally finished the game, but it's 11 years later.

But...Thief III's Cradle level had some disturbing enemies, like Puppet:

Just being in the Cradle was something.

Nobody yet?

Fine, the god damn Lickers from Resident Evil 2.

You hear them slipping on the floor, and breathing, they can be anywhere, on a wall, ceiling, around a corner. BEHIND YOU! =O then they have an insta-kill move ¬_¬ and finally they're bastards to hit, cause they lunge and jump all over the place, and if theres more than one of them...well good luck.

Plaster Splicers, after fighting them once I never used the Power to the People machine ever again. I waste all my ammo on anything that looks suspicous and I always seems to miss one and die. I mean they don't talk, or babble, or scream, or make any noise. It isn't fair to draw me into a false sense of security and pull that on me! Die you horrible horrible things. Oh and I also have a fear of inanimate things coming to life, so thats like the worst thing ever in a video game. Evar!

EDIT: Also Big Daddies, when you first play, nothing will prepare you for facing them. And when they pull out a new model, all hell breaks lose. Serious, I was mentally preparing myself to fight the one that charges (without knowing it charges) I'd just quick saved, I layed a few traps, pulled out my fully upgraded shotgun fired one shot... and Alt F4'd. Serious when he charged me I paniced screamed 'NOOOOO!' and just closed the game. It was too frightening to have one that could get into Melee Range. Don't mock me, I was young and this was the first game I ever played that I considered 'scary'

Scaarj Pupae, Unreal 1.

Small, jumpy things that get at your feet, difficult to hit, once they get too close they drop out of LoS, and even if you do notice them, they're bloody jumpy and difficult to hit at point blank.

They appear fairly early on, and throughout the game. Except they leave it until a late level where all the lights are out to give away their final ability - to stick to ceilings.

Ugly things, too.

The reason I've only completed Unreal 1 once, despite my love of the game.

'No one's here' D:
also, anyone in Ravenholm (except maybe Father Grigori) in Half-Life 2. so many enemies at once :(
also, I know it may seem a bit weak, but: those flood with rockets in Halo 1, on 'Two betrayals'. similar to those in 'the library' except with a ton of claustrophobic turns D:

The weird, zombie monster thing from Siren: Blood Curse.
Oh, and the spider zombies too, actually. They scare the hell out of me.

I hate enemies that can get back up again. Like the zombies from Blood Curse. There seems to be a recurring theme, here...

In terms of how badly something can affect your health...any poison-type Pokemon. If I'm not near a Pokecentre (and very rarely do I bother carrying Antidotes) then I panic, because I want my Pokemon to LOOOOOOOOOVE me.

The vampires from Vampire's Rain - terrible game? Yes. Invincible, fast moving vampires that will kill you with one hit? Also yes.

Oh, and Chainsaw Guy from Resident Evil 5. I HATE Chainsaw Guy. The sound of the saw as you get close is just...ARGH! I hate anything where you hear it before you see it, and you tense up because you're constantly expecting it, yet you don't know when it'll jump out.

That's the last one, I promise.

The Hunter from Dead Space. It doesn't matter how many times I've played the game, or how unscary the rest of the game has become for me at this point; the Hunter utterly terrifies me. At least he doesn't show up in Dead Space 2, right?

Wrong. Popping out of a vent completely unannounced in Chapter 14, the "Ubermorph" here does pretty much everything the Hunter did, like chasing you, growing back his limbs, trying to impale you, and generally being an asshole. My first thought when I first saw this thing was "Holy shit, is that an alien?!" and my second thought (when I realized its limbs were growing back) was "Oh, fuck me!" Then I ran away, because I didn't want to wait around for it to answer that question or comply with that request.
