In single player games that force me to provide a name for the main character with no default, I just borrow Snow Crash [http://en.wikipedia.org/wiki/Snow_Crash]'s main character's name: Hiro Protagonist.

On mutliplayer games I usually try and grab the same handle I'm using here.

I also wrote a name generator [http://www.xenoveritas.org/namegen] which can come in handy if something demands a name.
"Your Name: ?Eathanettennertith?"

I could use this for spore >_>

If it's a storyline character, as in it has a name already but they decided to give us a prompt anyway because what the fuck, I usually leave it as is. Doing otherwise breaks the immersion, and I am a big whore for atmosphere.

Other than that, I typically use my screenname. Mnemophage is gender-neutral and universally applicable to anything that is awesome.

I go for Captain in games when they are just setting up a name for the sake of setting up a name. Because I am the captain of your mind link! mwahaha!

In games where I might have a chance to develop a character as a character rather than just run around with the avatar they give me, I will typically name them based on what I want to do. I have 2 major names I use Kikyoo, for anytime I'm feeling incredibly psychotic and violent, and Shalindra for when I want a Female char who... I'm still fleshing out details for x.x.

If others will see it, it's my main screen name, "Sheepshifter." If no one else will see it, it's normally to deafult, but sometimes I'll name them really long ridiculous names like "Worthington"

Ronald Reagan (or some variation of Reagan) if it's a guy and Eleanor Roosevelt if it's a female (usually a variation of Eleanor).

I usually name a character the name they are "supposed" to have like Link in LoZ, but if they don't really have one or it just sucks I go for Klaus because I have always secretly wanted to be German or have a German/Dutch name. If pressed to tell why I suppose it'd be because I always saw Germany as a very efficient country and a polar opposite to Laziness, which I admire.

Met or M3t for online FPS games. If I'm playing singlea single player game, it's generally whatever the character's original name was, or Schlong or something similar to that.

I used to go with the generic character name for single player games, e.g. Link (or in a WoW induced parody, Linken), but then by sheer chance I hit on Siege, and it stuck.
It's nice to have a friendly number you can tack on to make sure you get that username you always wanted!

you were killed by a n00b

In Zelda games, I'll just stick with Link

For single-player games I choose pretty random names either based on the character's appearance or words that I think will be amusing when they pop up in dialog. Pokemon characters are usually 'Slut' or 'Manslut' (With Pokemon called 'Assmunch', 'Stripper' and 'Testacool', among others)
I have a cousin who does the same thing. Drives me crazy. I did a comic about it a while back.

I usually use Persephone Everett for female names and whatever comes to mind for male ones.

I try to give them a name that fits the universe they're a part of, especially if it's also a reference. My characters in Mass Effect are Jean {as in Jean-Luc Picard) and Lilith (Evangelion ftw).

When I get Final Fantasy XIII I'll take a random word that isn't a name and use that, because that's what Square Enix do.
