I've heard of that! It's on GOG, and I really want to play it (I've been indulging myself in bizzare point-and-click adventure games recently, like Beneath a Steel Sky, or I Have No Mouth, and I Must Scream). Unfortunately, it doesn't run on Windows 7.
OT: It'd have to be Blood, a 1990's FPS featuring the most charmingly insane main character of all time and a lot of really awesome, if somewhat sadistic, weapons.

The Otogi series for the Xbox and The Lost Kingdoms series for the Gamecube, both made by From Software. Beautiful games, it is still hard to find a action game where the character moves as swift and elegantly as Raikoh from Otogi.

It's not so much an obscure game, as the franchise it's built off of is still quite alive and well. But the 1993 Shadowrun for the SNES is still to this day one of my favorite games and one that I replay from time to time. I only with someone would make an RPG off of it again sometime soon.
I used to have a copy of the Genesis version. I do wish I had a working console for it... :\ Or you know, could still find the cartridge.

As for most obscure, probably Orb 3d for the NES. After that maybe Neverend for PC, though neither of those were exactly... well good. Tomb of the Taskmaker also comes to mind, though that was a Mac release... I wonder if I still have a copy of Bolo sitting on an old zip drive somewhere...

The X-Files: Resist or Serve for the Playstation 2. It's a lot better than you may believe, definitely worth picking up if you're into Survival Horror games.

The only truly obscure game I can think of is "Commando" for the PS2 (it may have had a subtitle but I sold it).

From what I could gleam from the 10 minutes I played it, it is basically a shitty RTS naval combat game.

Hammer & Sickle, You play as a Soviet agent infiltrating post war west-Germany and fight Nazi renegades, local thugs and American troops disguised as Russians. Its hard as hell and the mechanics are really unforgiving but I do occasionally love me some of that game.

Legend of Kay for the PS2. I never heard of that game before I bouhgt it at a second hand shop. It seems solid but I haven't played more than five minutes yet.

Obscure was quite obscure. As was its sequel, Obscure 2. :p

In any case, maybe Pathologic? That was an interesting game. Shame about the translation.

Little Ninja Brothers for the NES, an action RPG. With two player co-op!

It's aged decently and is one of the very few games of its time with a self-aware sense of humor.

The occasional traditional RPG-styled battles late in the game are total bullshit, though. You miss so often it's maddening, especially seeing as up to then, your ability to damage the enemy was never based on a random number generator.

I don't know why I even liked that game. I hate it now back in hindsight.

Anyways I don't really want to say about the crappy SNES liscenced game I own.

There are too many crappy liscenced games about that nobody knows about, so I would consider it cheating.

Bad Mojo - you play as a neurotic greedy skinny guy who turns into a cockroach and has to navigate through a vile apartment only to find out interesting stuff about his life, and get to know that the building is about to blow up. It's... weird but I like it.

Deadly Premonition - no need to introduce, I love this fucker to no end.

Dick Sucks: Terror In Titfield - yes, it exists... A sidescroller where you play as a penis commando avoiding condoms ant pairs of jugs, all disembodied of course. Eeeeeeeeehhhhh...

Yume Nikki - one of my favourites, you explore the dreams of an antisocial girl, and oh God what you find there.

You Have To Cut The Rope - a free title, just go play it and then shake your head in disbelief. It's umm, short alright.

The Binding Of Isaac - oh boy, this one's also very addictive.

Rez - some people call it art and a classic rythm game, but after playing some not-so-classic rythm games like the BIT.TRIP series, I am actually convinced that there exist rythm games that have something to do with rythm. Rez simply doesn't work for me, it's just weird. And the sequel was pffff, let's not even go into that. Oh, and let me remind you of:

The BIT.TRIP series - I actually suffered from optical illusions after playing RUNNER's longest level and trying to get all the gold for like an hour. When I looked away from my screen all the world around me seemed to... warp or something.

YARIS - a free XBLA title, and i still feel ripped off. At one point you are driving a Toyota Yaris with a mechanical arm attached to it that is able to shoot in a tube-like gravity altering racing track, racing against nothing and trying to avoid huge fat guys on a small bike who only wear a barely visible speedo. My. Fucking. God. Oh, and it's pretty much the worst XBLA title, if not the worst of all games in both of the XBOX consoles' lives.

The Legend Of Kage (not sure about the title...) - an NES game where you fly around (because fuck that if that's a jump there) killing the shit out of other flying ninjas, some of which can shoot fire out of their mouths.

Star Wars Chess - there's a nice collection of videos on youtube showcasing every "conflict animation". These include melting some players, beheading them ad other spicy things. You know, everything we loved in Star Wars and all.

Tomba and Alundra for the playstation 1 are both pretty fun. Tomba has pig wizards, can't really remember Alundra though.

Of this generation? Little King's Story for the Wii, I've seen like one other person mention it ever.

There is also a master system game about a girl in a red dress and you shoot the monsters around her, I can't even remember what it was called, I'm starting to think I imagined it.
